| Objective To investigate clinical feature, diagnostic examination, treatment and clinical outcome of patients with definite IE and to improve the diagnosis and treatment.Methods Retrospective study of28adults with definite IE who were admitted to Sir Run Run Shaw Hospital from June1,2009, through Juny1,2011and they were analysed on predisposing cardiac factor, clinical presentation, etiology echocardiography, in-hospital course and clinical outcome.Result The median age was50.9±17.4years and100%had native valve IE,with a male preponderance(1.33:1).Mitral prolapse was the most common predisposing cardiac factor (39.3%). Fever is the most common symptom (82.1%). Positive blood cultures were obtained in only36.4%of the cases, with streptococcus viridans being the most common organism. Echocardiography was diagnostic in96.4%of the patients. Vancomycin was most frequently utilized (53.6%).64.3%of the cases had at least1major complication.46.4%of the cases underwent surgery for IE. The in-hospital mortality was3.6%.Conclusion There are remarkable changes in predisposing disease and onset age Typical clinical presentations are rare. Pathogenic bacteria of IE had not greatly changed. The process of diagnosis and treatment should be improved. In conclusion, early diagnosis, timely surgery and appropriate utilization of antibiotics can improve the prognosis of IE patients. |
