The Therapeutic Effect Of Distal Femoral Varus Osteotomy For Valgus Osteoarthritis Of The Knees

Objective: Evaluate efficacy of the biplane close wedge distal femoral osteotomy(biplane CWDFO)for valgus knee with osteoarthritis in early stage.Methods : A retrospectively study was carried out on 33 cases(35knees)of biplane CWDFO treated in our department between 2015 and 2016.The preoperative and postoperative average hospital for special surgery knee scores(HSS scores),visual analogue scale scores(VAS scores),anatomy lateral distal femoral angles(aLDFA),anatomy femoral tibial angles(aFTA),the intersection of mechanical axis on tibial plateau,and bone healing status.Results: The preoperative and postoperative average aFTA were(159.74±7.22)and(176.68±2.98),the difference was significant(P < 0.01).The preoperative and postoperative average aFTA were(69.64±4.97)and(82.66±1.94),the difference was significant(P < 0.01).The postoperative HSS scores increased by 44.25±10.62,(t=16.670,P<0.01).The postoperative VAS scores increased by 5.75±3.34,(t=6.893,P< 0.01).The preoperative and postoperative intersection of mechanical axis on tibial plateau were(107.81±26.85)% and(42.59±4.04)%,(t=10.117,P<0.01).There was no correction loss,peroneal nerve injury,infection,deep venous thromboses or poor wound healing occur.Bone healing was satisfactory in all cases,without delay of bone union or non-union by follow-ups.Conclusion: Biplane CWDFO can correct lower limb alignment and retain the range of motion of knee with obvious curative effect.It makes sense for delaying the progress of knee osteoarthritis so far as to the artificial joint replacement.
