Objective: To explore the factors that affects the long term prognosis of posterior fosse hemangioblastoma. Methods: The clinical data of 40 cases with posterior fossa hemangioblastomas who underwent neurosurgical treatment in Shengjing Hospital,China Medical University, from January 2008 to September 2018,were collected, including age, gender, tumor size, tumor texture, tumor location, preoperative modified Rankin scale score, early postoperative modified Rankin scale score, and preoperative hydrocephalus indicator.According to the modified Rankin scale score(?2 or >2),these patients were divided into two groups: the group with good prognosis(23 cases)and the group with poor prognosis(17 cases). The univariate analysis and multivariate Logistic regression were used to analyze the factors influencing the long-term prognosis of posterior fossa hemangioblastoma after surgery. Results: In the total 40 cases,36 cases(90%)achieved total tumor resection.32 cases were followed up for more than 1 year.Among these cases,3 cases relapsed(7.5%).The tumor recurrence time was 41,48,and 60 months, respectively. In the total 40 cases,23 cases obtained good prognosis(mRS?2),17 cases obtained poor prognosis(mRS>2).Results of univariate analysis showed that, the tumor size, the tumor texture, the location of the tumor, the preoperative mRS score, and the postoperative mRS score, were significantly associated with long-term were significantly associated with long-term;however,the age, gender, and preoperative hydrocephalus, has not associated with long-term prognosis.Additionally, the results of multivariate logistic regression demonstrated that, the tumor location and the tumor texture were associated with long-term prognosis(P<0.05).Patients with tumor located in the front region of posterior fossa and solid tumor were found to have poor prognosis. Conclusion: the tumor location and the tumor texture were associated with long-term prognosis(P<0.05).Patients with tumor located in the front region of posterior fossa and solid tumor were found to have poor prognosis.
