Objective: To study the general data and clinical features of posterior fossa hemangioblastoma,and to explore its diagnosis and treatment methods in order to improve the therapeutic effect.Methods: We retrospectively analyzed the general data,clinical manifestations,imaging data,and methods of treatment for 74 cases of posterior hemangioblastoma treated in our hospital from December 2009 to December 2017.Results: 46 males and 28 females,male: female = 1.65:1;aged 20 to 75 years,median age40 years old.The disease duration ranged from 4 days to 6 years.The average disease duration was 6 months.There was no correlation between tumor volume and disease duration.There were 59 cases of cystic nodular HB,of which 9 cases were multiple,and50 cases were single;there were 15 cases of substantial HB,of which 1 case was multiple,and 14 cases were single;a total of 93 tumors.Substantial HB occurs in the medulla oblongata,the fourth ventricle,and the cerebellopontine angle region.Cystic nodular HB often occurs in the cerebellar hemisphere and the cerebellar vermis.All 74 patients underwent surgical treatment and 74 tumors were resected.Ten patients with intracranial multiple HB failed to remove all lesions.Complete resection was performed in 50 patients with single cystic nodular HB and complete resection was performed in one case.Substantial HB was performed in 11 cases and subtotal resection in 3 cases.The total cystectomy rate of cystic nodular HB was higher than that of substantial HB.Postoperative cranial nerve palsy in 7 cases,postoperative complication rate was 27.03%.72 patients were followed up for a period of 3 months to 7 years with an average of 12 months.The KPS score was followed up for 6 months after surgery.KPS ? 80 points in 72 patients.Conclusion: CT and MRI are necessary examinations for the diagnosis of hemangioblastoma.Microsurgery is the best treatment option for total tumor removal.
