The Clinical Analysis Features Of Adult Autoimmune Encephalitis

Objective:Through the analysis of the clinical manifestations of autoimmune encephalitis,characteristics of epileptic seizures,EEG characteristics and imaging changes,to improve clinicians'understanding of the disease.Methods:Clinical manifestations,laboratory examination,imaging and electroencephalogram,treatment and outcome with autoimmune encephalitis were retrospectively analyzed,who were admitted to the department of neurology of shengjing hospital affiliated to China medical university from April 2016 to January 2019.Results:A total of 38 patients were included,including 23 males and 15 females,the ratio of male to female was 1.5:1.The age of onset was(45.95±20.42)years,and the average hospital stay was 19.76 days.There were anti-NMDAR encephalitis in 21patients,LGI1 encephalitis in 12,anti-GABA_BR encephalitis in 5.89.47%of the patients had epileptic seizures,mainly generalized onset,and 17 patients showed non-specific abnormal electroencephalogram,epileptiform discharge in 7 patients.The involved parts of head MRI were mainly the medial temporal lobe and hippocampus,basal ganglia,frontal parietal occipital lobe,cerebellum and corpus callosum are rare.34 cases received immunotherapy,21 cases received intravenous immunoglobulin,31 cases received hormone therapy,and 5 patients received long-term immunotherapy.31 patients with seizures received antiepileptic drugs,19 patients were effectively controlled.35 patients were improved after immunotherapy and other symptomatic treatment,including 1patient who relapsed more than 2 months after discharge,and 1 patient who developed iatrogenic steroid diabetes during oral glucocorticoids treatment.Conclusion:Different types of autoimmune encephalitis have different clinical characteristics.Combined with clinical manifestations and auxiliary examinations,early identification and immunotherapy were performed,and the prognosis was good.
