Retrospective Study On Etiology, Pathogenesis, Seizure Type And Antiepileptic Drugs In Adult Patients With Epilepsy Quality Control Platform

Objective:According to the clinical data of epilepsy quality control platform,the causes and types of epileptic seizures in this region are summarized,and the usage and clinical efficacy of antiepileptic drugs are analyzed and discussed,so as to provide basis for reasonable treatment of epilepsy drugs in clinical practice and help clinicians to select antiepileptic drugs correctly,reasonably and scientifically.Methods:The clinical data of epileptic patients diagnosed in our hospital from November 2017 to December 2019 were collected.All patients met the2014 epileptic diagnosis standard of the International Anti-epileptic Alliance.Senior neurologists evaluated the past history,etiology,seizure types and antiepileptic drugs of epilepsy cases.The basic information and clinical data of the study subjects were analyzed: the basic information of the patients(age,sex,past medical history,family history,etc.),and the clinical data included(seizure time,seizure type,examination results,medication situation,prognosis,etc.).Excel was used to input and sort out the data of epilepsy platform,SPSS 20.0 was used for statisticalanalysis,percentage was used to describe the data,?2 test was used to compare the data between groups,P<0.05 showed that the differences between groups were statistically significant.Results:1.General situation: 300 male patients(67.87%)and 142 female patients(32.13%).The ratio of male to female is 1.43: 1.The average age of the patients was 24.33±19.39,of which the patients aged 0-40 were the most,accounting for 52.26% of the total number.135 patients aged 0-10,accounting for 30.54%;96 people aged 11-20,accounting for 21.72%;76people aged 21-30,accounting for 17.19%;60 people aged 31-40,accounting for 13.57%;27 people aged 41-50,accounting for 6.11%;44people over 51 years old,accounting for 9.95%.2.Etiology: Of 442 patients,250(56.56%)had uncertain causes and192(43.44%)had definite causes.Epilepsy caused by brain injury accounted for the highest proportion(30.21%),followed by epilepsy caused by intracranial infection accounted for 17.19%,epilepsy caused by birth injury and family history accounted for 15.10% and 10.42%,respectively.Cerebrovascular diseases and brain tumors account for5.20% and 6.25% respectively,and 14.06% of the patients have epileptic seizures caused by other reasons,including brain atrophy,non-ketotic diabetes,toxic encephalopathy,etc.3.Attack type: The proportion of focal attacks is the highest,with206 persons,accounting for 46.61% of the total number of attacks,followed by 178 persons with systemic attacks,accounting for 40.27% of the total number of attacks,and some patients with unknown attacks,with58 persons,accounting for 13.12% of the total number of attacks.4.Drug treatment status: 289 patients were newly diagnosed,accounting for 65.38% of the total number of patients.The analysis of medication shows that the vast majority of patients are treated with single medication,accounting for 69.91% of the total number of patients,and128 patients are treated with combined medication,accounting for28.96%.Among them,167 patients were treated with traditional antiepileptic drugs,accounting for 54.05%.132 patients were treated with new antiepileptic drugs,accounting for 42.72%.Analysis of patients taking combined drugs found that more than half of the patients were treated with traditional drugs combined with new drugs,accounting for56.25%,26.54% were treated with combination of two traditional drugs,and 14.84% were treated with combination of two new drugs.5.Drug treatment of patients with different epileptic seizure types:The number of patients with comprehensive origin and unknown origin was significantly higher than that of patients with focal origin,and patients with focal origin were significantly higher than those of the other two groups.The utilization rate of traditional drugs in patients withsystemic attacks is significantly higher than that in patients with focal attacks and unknown attacks.The utilization rate of new drugs in patients with focal attacks is significantly higher than that in patients with systemic attacks and unknown attacks.However,the ratio of traditional drugs combined with new drugs in patients with unknown attacks is far higher than that in patients with systemic attacks and patients with local attacks.Conclusion:1.Male patients are more than female patients,of which the patients aged 0-40 were the most.2.More than half of the patients have no definite cause.Brain trauma accounts for the highest proportion of patients with definite cause,followed by intracranial infection,birth injury and family genetic factors.3.The seizure type is mainly focal seizure,followed by systemic seizure and unknown seizure.4.Single drug therapy is the main medication method for epilepsy.The usage rate of traditional drugs is significantly higher than that of new anti-epileptic drugs.The combined drugs are mainly traditional drugs and new drugs,followed by traditional drugs and traditional drugs.5.The number of patients with comprehensive origin and unknown origin combined
