Analysis Of Factors Influencing Blood Pressure Control In Hypertension Patients And Establishment Of Pharmaceutical Service Path

Objective: Starting from patients and doctors,through the analysis of influencing factors and the method of evidence-based pharmacy,this paper explores the main points of pharmaceutical care for hypertension,assists clinicians to control blood pressure comprehensively,and improves the comprehensive management level of hypertension.Establishing the pharmaceutical care path for hypertension and providing theoretical basis for clinical pharmacists to carry out standardized pharmaceutical care.Methods: The study is divided into two parts.The first part is divided into two levels to analyze the influencing factors of blood pressure control.For patients,the questionnaire of influencing factors of hypertension control was formulated and filled in,and the main factors influencing blood pressure control were screened by single factor chi-square test and multifactor logistic analysis.According to the doctor's part,by searching the relevant websites of clinical practice guides such as National Guideline Clearinghouse(NGC),Guidelines in Britain,PubMed,China Knowledge Network and other websites,we summarize and analyze the treatment objectives and medication plans of the latest hypertension-related guides in various countries,and evaluate the clinician's guidelines Compliance on the basis of these websites to explore medical factors that may lead to poor blood pressure control.In the second part,according to the main factors affecting blood pressure control in patients,we should adjust the direction and key content of pharmaceutical care and medication education,study the problems exposed according to doctor's guideline compliance,lock in the content of medical services for doctors,combine the concept of integrated health care system for hypertension,introduce new means such as electronic propaganda and Wechat interaction,and establish standardized pharmacy service path.Results: The results of influencing factors showed that poor compliance with medication,smoking,salty diet,eating shepherd's oil,long sitting time and mental anxiety and insomnia were the main causes of poor blood pressure control,and also the focus of medication education.The results showed that 178 patients complied with the guidelines and the overall compliance rate was 71.77%.169 patients met the blood pressure standard at discharge,the compliance rate was 68.14%.The correlation analysis showed that the compliance rate was 2.34 times and 95% confidence interval was(1.319,4.317)compared with the patients who did not comply with the guidelines.Conclusion: Based on the analysis results of influencing factors of blood pressure control,assisting clinicians to formulate rational drug delivery plans and solving practical clinical problems,a more targeted and feasible pharmaceutical monitoring path is developed to provide services for patients and doctors,and to ensure the safety and rationality of patients' medication.
