Background: Prostate cancer is the second most common diagnosed cancer in men.Due to the low specificity of current diagnosis methods for detecting prostate cancer,patients with prostate disease are often over diagnosed and treated,which bring unnessarty pain to the patients.So the identification of new biomarkers is highly desirable.The study was conducted to determine the clinical utility of the PSEP(prostatic exosomal protein)in the aided diagnosis of prostate cancer.Method:The study included men who had a biopsy in Renji hospital from October 2016 to January 2017 due to abnormal PSA.We collected their urine and stored in the refrigerator at-80 DEG C.The urines were tested within 2 months or immediately detected.We evaluate the accuracy of PSEP in the aided diagnosis of prostate cancer by calculating the sensitivity and specificity of PESP in diagnosis of prostate cancer in the patients with abnormal PSA.Results:There was significant difference between Prostatitis patients and non-prostatitis patients in the experimental group.According to the results of PSEP and the diagnosis of prostate cancer,we drew a ROC curve.The area under the curve is 0.806.We can conclude that urinary PSEP has diagnostic significance in prostate disease patients with abnormal PSA,.Itcan improve the diagnostic specificity of prostate cancer.According to the Yuden index to choose the maximum valuecorresponding to the point as the standard of diagnosis of prostate cancer.The value of cutoff is 1.2335ng/ml with the sensitivity of 81.8%and the specificity of 70.6%.Conclusion:Due to the low Specificity of PSA for the diagnosis of prostate cancer,the unnecessary pain of patients are increased.PSEP can significantly improve the diagnostic rate of prostate cancer in the prostate disaeas patients with abnormal PSA and reduce invasive prostate biopsy and over treatment.It also reduce the misdiagnosis rate in early detection screen for prostate cancer.Research should be explored in future study to determine the prediction value in the aided diagnosis of prostate cancer.
