Objectives:To study the application value of PSEP in the diagnosis of NIH-?prostatitis,we collected and detected the prostatic body leakage protein(PSEP)in urine, and analyzed the correlation between PSEP and the routine indexes of prostatic massage fluid(EPS).Methods:The subjects were 159 patients who met the inclusion criteria from August 2019 to March 2020,including 43 healthy men,61 NIH-? a prostatitis patients and 55 NIH-? B prostatitis patients. The urine and EPS of the subjects were collected, the content of PSEP in urine was determined by enzyme-linked immunosorbent assay(ELISA),and the white blood cells and lecithin counts were obtained by detecting EPS under the microscope. The differences of urine psep content among the three groups were compared to obtain the specificity and sensitivity of PSEP detection. The content of PSEP in NIH-? prostatitis was correlated with leukocyte count and lecithin count in EPS.All the above operations and laboratory tests are performed by experienced doctors and medical technicians to reduce measurement data bias.Results:1 PSEP test data1.1 The content of PSEP in urine of normal male group,NIH-? a group and NIH-? B group were all non normal distribution, the values were 0.62(0.11-1.79)ng / ml,1.85(0.90-6.51)ng / ml and 1.97(0.79-6.45)ng / ml respectively. The content of PSEP in urine of experimental group and control group was significantly different(P < 0.01).1.2 The sensitivity and specificity of PSEP were 93.4%,73.8% and 88.1% respectively.1.3 The area of ROC curve of PSEP in urine of patients with NIH ? A and NIH ? B prostatitis was 0.751 and 0.809 respectively. The point with the largest Youden index was selected as the diagnostic standard of CP,which was 1.15ng/ml and 1.18ng/ml respectively. The sensitivity was 88.5% and 90.9%,and the specificity was 59.2% and 61.3%,respectively.2 Correlation Analysis of PSEP and EPS Routine Indexes2.1 There was a significant positive correlation between PSEP in urine and leukocyte count in EPS(r = 0.642,P < 0.001).There was a low positive correlation between PSEP in urine and leukocyte count in EPS(r = 0.195,P = 0.153).2.2 There was no significant correlation between PSEP in urine and lecithin count in EPS in NIH ? a CP Group(r = 0.145,P = 0.265),and there was a low degree negative correlation between PSEP in urine and lecithin count in EPS in NIH ? B CP Group(r =-0.111,P = 0.483).Conclusions:1 The content of PSEP in urine of NIH-? prostatitis patients was significantly higher than that of the control group, which had higher sensitivity and specificity, suggesting that PSEP can be used for clinical diagnosis of chronic prostatitis.2 The differences of leukocyte count and lecithin index between PSEP and EPS were found in NIH-? a prostatitis group and NIH-? B prostatitis group.3 The white blood cell count and lecithin count in prostatic fluid can't be regarded as the diagnosis standard of chronic prostatitis, and should be considered with clinical symptoms, psychological factors and so on.
