| Objective To investigate the relationship between severity of osteoporosis(OP)in elderly men with chronic obstructive pulmonary disease(COPD).Methods 50 patients with COPD in the elderly in Xining were selected as the COPD group,and 30 healthy elderly men were used as the control group.The bone mineral density(BMD)was measured by dual energy X-ray.The CAT score,mMAC grade,acute exacerbation history and general condition of COPD group were recorded.According to the results,the COPD components were group of GOLD A,B,C and D,and the lung function of the case group was determined(FEVi accounted for%of predicted value,FEVi/FVC).According to the result,the COPD component is of the GOLD 1,2,3,and 4 grades.The serum levels of osteocalcin(OC)and quinoid collagen cross-linked carboxy-terminal peptide(CTXⅠ)in the control group and COPD group were detected by ELISA.The bone mineral density values of the two groups and the COPD group serum OC and CTXⅠ levels in each grade were compared and correlated.Results COPD group abnormal bone mass ratio 90%(45/50),osteoporosis(OP)detection rate 36%(18/50),compared with the control group[(70%,21/30)(13.3%,4/30)]significantly increased in the COPD group,lumbar spine BMD(-1.84± 1.34),femoral neck BMD(-1.70±0.82),much lower than the control group[(-1.14±1.08),(-0.92±0.73))],the difference was statistically significant(P<0.05).The control group formed a group control with GOLD A,B,C,D group and GOLD 1,2,3,and 4 levels.The BMD and OC concentrations of lumbar vertebrae and femoral neck showed a decreasing trend,while the concentration of CTXⅠ showed an increasing trend.Academic significance(P<0.05).The COPD group CAT score was linearly negatively correlated with OC concentration,femoral neck and lumbar spine BMD(r values were equal to-0.6,-0.53,-0.59,respectively),and was positively correlated with CTXⅠconcentration(r value was 0.57).FEVi%pred and FEVi/FVC%in COPD group were positively correlated with OC concentration value,femoral neck and lumbar spine BMD(r values were 0.58,0.73,0.69,0.54,0.54,0.55,respectively),and were linearly negatively correlated with CTXⅠ concentration(The r values were-0.74,-0.71);the CAT scores continued to be negatively correlated with the OC concentration values(r value was-0.37),and the FEVi%pred,FEVi/FVC and CTXⅠconcentrations were significantly negatively correlated(r=-0.41,-0.37).Conclusion The abnormal rate of bone mass and the detection rate of OP in patients with COPD were significantly higher than those in the control group,while the BMD value was significantly decreased.In the early stage of COPD,the BMD value and bone metabolism in the body were not significantly changed.With the aggravation of COPD patients,BMD showed a gradual decline.Trends;CAT scores and pulmonary function results(FEV1%pred,FEV1/FVC%)in COPD patients were correlated with serum OC and CTXⅠ concentrations. |
