Objective: Objective: To investigate the value of CT spectroscopy in the diagnosis and differential diagnosis of cirrhosis-related nodules.Methods: 50 patients with cirrhosis and intrahepatic nodules were scanned by CT spectroscopy.40 patients with cirrhosis-related nodules were diagnosed by energy spectrum analysis,including 13 RN nodules and 5 LGDN nodules.There were 10 cases of HGDN nodules and 12 cases of SHCC nodules,and the remaining 10 cases were non-cirrhosis related nodules.All nodules were confirmed by pathology,and 41 cases of cirrhosis-related nodules confirmed by pathological examination were finally screened and divided into 4 groups,14 cases of RN,7 cases of LGDN,9 cases of HGDN,and 11 cases of SHCC.Sexual analysis of CT spectroscopy images of four different pathological types of cirrhosis-related nodules,observing the trend of energy attenuation curve of each group of cirrhosis-related nodules,calculating the optimal contrast-to-noise ratio(CNR)and the best single energy value,The average iodine concentration in the nodules of two different cirrhosis-related nodules was measured and compared during the same period and the normalized iodine concentration was calculated.Results: The CT values of cirrhosis-related nodules decreased with energy at 40-140 keV,and the optimal contrast-to-noise ratio(CNR)of cirrhosis-related nodule lesions was found to be between 50 and 70 keV.The energy value is 70 keV.The average iodine concentration values of the nodules in each group were compared.There was no significant difference in the iodine concentration between the HGDN group and the SHCC group(P>0.05).There were differences in the iodine concentration between the other groups in the arterial phase,portal vein phase and delayed phase.Statistical significance(P <0.01).Standardized iodine concentration(NIC)of each group of nodules: RN group and LGDN group,RN group and HGDN group,RN group and SHCC group,LGDN group and HGDN group,LGDN group and SHCC group,HGDN group and SHCC group The arterial phase,portal vein phase and delayed phase NIC were statistically significant(P<0.01).Conclusion: CT energy spectrum single energy map can clearly show the lesions and increase the detection rate of cirrhosis-related nodules.The iodine-based image of CT energy spectrum can quantitatively distinguish liver regeneration nodules,atypical hyperplasia nodules and small liver cancer nodules.The sensitivity of CT spectroscopy for the diagnosis of cirrhosis-related nodules(true positive rate)was92%;the specificity(true negative rate)was 77%,and the Yoden index was 0.69.It is proved that CT energy spectrum imaging has certain value for the display,quantitative qualitative diagnosis and differential diagnosis of cirrhosis-related nodular lesions.
