Study On The Risk Of Hepatocellular Carcinoma In Hepatitis B Cirrhosis Combined With Type 2 Diabetes Mellitus

Abstract/Summary:
Objective To determine the association between HCC risk and type 2 diabetes mellitus,and the effects of diabetes duration and types of diabetes treatment on the risk.Methods We conducted a retrospective case-control study.Firstly,325 cases of hepatitis B liver cirrhosis complicated with HCC were selected as case group and 601 cases of hepatitis B cirrhosis as control group,multivariate Logistic regression analysis was used to explor correlative factors with the risk of HCC.Secondly,seleting patients with type 2diabetes mellitus both in the case group and the control group,multivariate Logistic regression analysis was used to explore correlative factors of diabetes mellites with the risk of HCC.Result The prevalence of diabetes mellitus in Hepatitis B cirrhosis with HCC group was 14.2%and 6.0% in hepatitis B cirrhosis group.Multivariate Logistic regression analysis showed that type 2 diabetes mellitus(AOR = 1.982.95 CI: 1.55-3.04)was a risk factor for HCC,increased the risk of HCC.Compared with those whose daisease couese of diabetes mellirus < 10 years,the risk of HCC was significantly increased in those ≥10 years(AOR=6.011,95%CI:1.659-21.777).Metformin(AOR=0.188,95%CI:0.052-0.688)reduced the risk,and insulin(AOR=6.682,95%CI:1.899-23.510)increased the risk.Conclusion Type 2 diabetes mellitus is one of the independent risk factors for HCC,which can significantly increase the risk of HCC,and the risk is related with disease course of diabete mellitus,the course of diabete mellitus≥10 years increases the risk of HCC,Metformin reduces the risk.
