Objedtive To investigate whether type 2 diabetes mellitus(DM2) could increase the morbidity of the hepatocellular carcinoma(HCC) and whether type 2 diabetes were an independent risk factors to the HCC.Methods From January 1st 2002 to January 1st 2010,we conducted a case-control study to analyse the effect of DM2 on the incidence of HCC. We conducted a logistic regression analysis to analyse the effect of age, sex, DM2 and coronary heart disease(CHD) on the incidence of HCC. Expound the oncogenic mechanisms by which diabetes predisposes to HCC, through statistics analysis the course of DM2 and the level of fasting blood-qlucose(FBG).Results The odds ratio(OR) of HCC in patients who have diabetes was 1.50(P<0.05). Among the male patients of 1690 HCC cases, the OR of HCC in patients who have diabetes was 1.50(P<0.05), and the OR of HCC in female patients who have diabetes was 1.52(P<0.05). The OR of HCC in patients who have diabetes but without chronic hepatitis B(CHB) was 1.50(P<0.05). Multiple factor regression analysis displayed that DM possess significant effect on HCC, the OR of HCC was 1.544(P<0.05). DM2 pre-exists HCC development for a mean time of 54.37±48.66 months. 80% of HCC diabetic patients'fasting blood-qlucose was higher than normal: 9.34±4.55mmol/L. About 66.67% of HCC diabetic patients were treated with exogenous insulin or sulfonylureas. Only 11.46% of HCC diabetic patients were treated with metformin.Conclusion Patient with DM2 are at increased risk of HCC, DM2 was independent risk factor of HCC. The mechanism of DM2 cause HCC may: 1.insulin-resistant and hyperinsulinemic;2.high blood glucose; 3.nonalcoholic stesatohepatitis(NASH).
