Objective:1、The paper is to reach the correlation between Type 2 diabetes mellitus and hepatocellular carcinoma with chronic hepatitis B virus.2、To further explore whether type 2 diabetes plays the same role in HCC development in cirrhotic and non-cirrhotic patients with CHB.Methods:We will using case-control study. The researched cases come from the second affiliated hospital of shanxi medical university and the tumor hospital of Shanxi Province from January 2012 to October 2015.Subjects who fulfilled the following criteria were recruited into the study : hospitalized for HCC or CHB, age>=30years,positive for hepatitis B surface antigen(HBs Ag(+)), negative for anti-HCV,without a history of drinking(alcohol standard: > = 40 g/d, men women > = 20 g/d,and 5 years; or drink 80 g/d > 2 weeks.),without a history or other evidence of cancer other than HCC,without a history or other evidence of hepatitis other than hepatitis B,the han nationality and the residents in Shanxi Province.A total of 223 eligible consecutive patients,101 HBV-related HCC and 122 CHB without HCC, were enrolled in the study.A total of 101 HBV-related HCC patients were used as the case group in this study.The 122 CHB patients without HCC were used as the contron group.Results:1、Patient characteristics:Most patients were men in the two groups, and the proportion of case group is higher than the control group(85.15% vs 70.49%).The proportion of cirrhosis in case group is significantly higher than the control group(79.21% vs 32.79%).Ppatients of case gruop were older than control’s;the mean age±standard error was 54.42±9.21 vs 45.98±10.61,the median age was 54 vs45,respectively.The distribution of family history of liver cancer in cases group is more than control’s(12.87% vs 4.92%); The prevalence of diabetes mellitus were higher among cases than among controls(19.80% vs 9.84%).The prevalence of HBV-DNA>105copies/m L were lower among cases than among controls(34.65%vs 50.00%).2、The results of Single factor analysis with chi-square test:The chi-square test indicated thatsex, age, history of type 2 diabetes, liver cirrhosis, family history of liver cancer and HBV-DNA loads are the risks of HCC related HBV occurrence(P <0.05). The corresponding chi-square value are 6.729, 55.327, 4.465, 47.907, 4.484,47.907,respectively.3、The results of Multiariable logistic regression analysis: Multiple logistic regression analyses indicate that patients’ sex, age, history of type 2 diabetes, liver cirrhosis, family history of liver cancer and HBV-DNA loads are the risks of HCC wiht CHB; The corresponding OR are 3.381, 0.854, 4.528, 3.345, 7.973,3.345,respectively.4、The characteristics after stratification according to cirrhosis: After stratification according to cirrhosis,we can see that the age has obvious difference among the two groups(P<0.001).Although the proportion of history of diabetes in the cases is higher than the control group, the chi-square test indicate that the value isn’t statistical significance.5、The results of Multiariable logistic regression analysis results after stratification according to cirrhosis:Multiple logistic regression analyses indicate that patients’ sex, age, history of type 2 diabetes, family history of liver cancer, HBV-DNA loads are the risks of HCC wiht hepatitis cirrhosis.However,we can see that the history of type 2 diabetes and sex have not correlation with the incidence of HCCwithout hepatitis cirrhosis.Conclusion:1、Type 2 diabetes may be exist positive correlation in the occurrence of HCC with Hepatitis B virus.2、Liver cirrhosis may play a collaborative role with type 2 diabetes in the occurrence and development of HCC,for CHB patients.3、Men, family history of liver cirrhosis, family history of liver cancer and HBV-DNA loads(> 100000 copies/ml) are risk factor for HBV related HCC.
