ObjectiveTo investigate and analyze the current situation of the self-care agency of patients with temporary enterostomy and to based on Management By Problem to formulate the self-care health education program,implement the intervention program and evaluate the application effect.MethodsAccording to the steps of the Management By Problem,questionnaire survey was adopted to find out the current situation of self-care for patients with temporary enterostomy,analyzing the existing problems of health education,and to formulate and implement corresponding intervention programs.To select 70 patients with rectal cancer who were admitted to the Colorectal Surgery Department of General Hospital of Ningxia Medical University for temporary enterostomy.The samples were divided into control group and intervention group.The control group received routine health education,while the intervention group carried out health education guided by Management By Problem.The scores of self-care agency,quality of life and qualified rate of bag replacement,complications,satisfaction with nursing work were compared and analyzed after the intervention.Results1.A total of 67 subjects completed the intervention,including 34 in the intervention group and 33 in the control group.There were 38 males and 29 females,with an average age(59.42±8.75)years old,with a minimum age of 28 years old and a maximum age of 76 years old.There was no significant difference in general data between the two groups(P >0.05).2.The two groups before discharge,1 month,3 months after the operation,the total scoreof ESCA and each dimension score of intervention group were higher than control group,the difference was statistically significant(P < 0.01),repetitive measure analysis of variance at each time point,there were significant differences in the time-effect and inter-group effects between the total scores and each dimension scores of the self-care agency(P < 0.01),and there was an interaction effect between groups and time factors(P?0.05).3.The two groups before discharge,1 month,3 months after the operation,the total score of C-COH and each dimension score difference was statistically significant(P?0.05),repeated measurement at each time point,there were significant differences in the time-effect and inter-group effects between the total scores and each dimension scores except for the physiological health dimension of the quality of life(P?0.05),and there was an interaction effect between groups and time factors(P?0.05).4.Before discharge,the qualified rate of bag replacement in the intervention group was higher than that in the control group,and the difference was statistically significant(P<0.01).5.At 1 and 3 months after surgery,the incidence of ostomy complications in the intervention group was lower than that in the control group(P?0.05).6.After the end of the intervention,the nursing satisfaction of the intervention group was higher than that of the control group,and the difference was statistically significant(P<0.01).ConclusionThe implementation of the Management By Problem of health education can improve the self-care agency and quality of life of patients with temporary intestinal ostomy,improve the qualified rate of pocket replacement,reduce the incidence of complications of ostomy,and increase the satisfaction of patients with nursing work.
