Objective:It is to study the relationship between TCM syndrome characteristics and adipokines expression and estrogen receptor expression in patients of perimenopausal obesity with phlegm syndrome of liver-qi stagnation.And to explore the differences in the expression levels of adipokines,estrogen and estrogen receptors in patients of perimenopausal obesity with phlegm syndrome of liver-qi stagnation and non-liver-qi stagnation.To provide direction for the clinical use of traditional Chinese medicine to treat perimenopausal obesity with phlegm syndrome of liver-qi stagnation patients,and to provide an objective and reference basis for the treatment of perimenopausal obesity with phlegm syndrome of liver-qi stagnation with traditional Chinese medicine.Methods:According to TCM syndrome differentiation method,volunteers from the Health Management Center of the Third People's Hospital affiliated to Fujian University of Traditional Chinese Medicine were collected and divided into 114 groups of perimenopausal obesity with phlegm syndrome of liver-qi stagnation group,96 perimenopausal obesity with phlegm syndrome of non-liver-qi stagnation,and 104 perimenopausal normal group.The distribution characteristics of the syndromes in the perimenopausal obesity with phlegm syndrome of liver-qi stagnation group and non-liver-qi stagnation group were analyzed,and simple body fat parameters and clinical blood collection were collected.Determination of adiponectin,leptin and estrogen by ELISA;the protein expression of ER? and ER? was detected by Western blotting.the gene expression of ER? mRNA and ER? mRNA was detected by RT-PCR.Results: 1.The visceral fat area of perimenopausal obesity with phlegm syndrome of liver-qi stagnation group was significantly higher than that of non-liver-qi stagnation group and perimenopausal normal group(P<0.01,P<0.05);2.Comparison of the characteristics of the syndromes of perimenopausal obesity with phlegm syndrome of liver-qi stagnation group and non-liver-qi stagnation group syndrome: liver and spleen,kidney,uterus,stomach,lung,heart,muscle and bone disease syndrome,as well as qi stagnation,dampness,heat,blood stasis,yin deficiency,viral syndrome of qi deficiency and impotence,the frequency and integral were significantly higher than those in the perimenopausal obesity of non-liver-qi stagnation group(P<0.01,P<0.05);3.The characteristics of mixed syndromes of perimenopausal obesity with phlegm syndrome of liver-qi stagnation: the main mixed disease locationa are spleen,kidney,and uterus;the main mixed excess disease are heat,dampness and blood stasis;the main mixed deficiency disease nature are yin deficiency and qi deficiency;4.Comparison of adipokines between the perimenopausal obesity with phlegm syndrome of liver-qi stagnation group and non-liver-qi stagnation group:there was no significant difference between adiponectin and leptin in liver-stagnation group and non-liver-stagnation group,but the adiponectin level in the two groups was significantly lower than that in the normal group(P<0.01,P<0.05).Leptin was significantly higher in the two groups than in the normal control group(P<0.01,P<0.05);5.The relationship between the main mixed syndromes in the perimenopausal obesity with phlegm syndrome of liver-qi stagnation group: the content of adiponectin in palace was higher than that of non-palace(P<0.01,P<0.05),the content of leptin in spleen was higher than that of non-spleen(P<0.01,P<0.05),The content of leptin in patients with kidney was higher than that in non-renal(P<0.01,P<0.05),the content of leptin in patients with yang deficiency was higher than that in non-yang deficiency(P<0.01,P<0.05),the content of leptin in patients with mixed blood deficiency was higher than that in non-blood deficiency(P<0.01,P<0.05),the content of leptin in patients with in palace was lower than that in non-palace(P<0.01,P<0.05);6.Comparison of estrogen content between the perimenopausal obesity with phlegm syndrome of liver-qi stagnation group and non-liver-qi stagnation group:there was no significant difference in estrogen between in liver-stagnation group and non-liver-stagnation group,but estrogen was significantly lower in the two groups than in the normal group(P<0.01,P<0.05);7.Comparison of estrogen receptor protein and mRNA content between the perimenopausal obesity with phlegm syndrome of liver-qi stagnation group and non-liver-qi stagnation group: the expression of estrogen receptor protein and estrogen receptor mRNA in perimenopausal obesity with phlegm syndrome of liver-qi stagnation is basically the same.There was no significant difference in ER? protein between perimenopausal obesity with phlegm syndrome of liver-qi stagnation group and non-liver-qi stagnation group,but ER? mRNA was significantly higher than non-liver-qi stagnation group,and ER? expression in liver-qi stagnation group was significantly higher in normal control group(P<0.01,P<0.05);the relationship in perimenopausal ER? protein and ER? mRNA expression was significantly different between phlegm syndrome of liver-qi stagnation group,phlegm syndrome non-liver-qi stagnation group and normal group,it increased in turn(P<0.01,P<0.05)Conclusions: 1.Liver-qi stagnation pathology affects the distribution of body fat in perimenopausal obesity with phlegm syndrome,liver-qi stagnation is a pathological factor that forms central obesity in perimenopausal obesity with phlegm syndrome;2.The characteristics of mixed syndromes of perimenopausal obesity with phlegm syndrome of liver-qi stagnation: the main mixed disease locationa are spleen,kidney,and uterus;the main mixed excess disease are heat,dampness and blood stasis;the main mixed deficiency disease nature are yin deficiency and qi deficiency;3.The expression of leptin is significantly different from that of adiponectin in perimenopausal obesity with phlegm syndrome of liver-qi stagnation of The characteristics of mixed syndromes,in the case of spleen,kidney,yang deficiency,and blood deficiency,the content of the syndrome is higher than that of the non-mixed spleen,and the content of leptin is also low,adiponectin is high in the cell of the peri-menopausal obesity and phlegm-stagnation syndrome,there is no difference in other syndromes;4.Liver-qi stagnation pathology affects estrogen receptor beta expression in perimenopausal obesity with phlegm syndrome patients.
