Objective:To compare the postoperative analgesia effects of ultrasound-guided serratus plane block combined with brachial plexus block,simple serratus plane block and patient controlled intravenous analgesia for mammary cancer,and to explore the clinical value of serratus plane block combined with brachial plexus block in patients with modified radical mastectomy for breast cancer.Method:60 patients with modified radical mastectomy for breast cancer were selected,age was 30 ~ 65 years old,ASA was grade I or II.The cases were randomly divided into three groups(n = 20): control group:the group with patient controlled intravenous analgesia(PCIA group);the group with serratus plane block(SPB group);the group with serratus plane block plus brachial plexus block(SPB +Bgroup).All three groups received intravenous combined inhalation anesthesia.In SPB group,the serratus plane block were performed under ultrasound guidance before anesthesia induction.In SPB + B group,the serratus plane block and brachial plexus block were performed under ultrasound guidance before anesthesia induction.The PCIA group was given intravenous analgesia pump(sufentanil 100 ug + 98 ml normal saline).Record:1.Time of anesthesia recovery and extubation time.2.At 6h,12 h,24h after operation,the VAS scores of the patient under resting and exercise conditions were collected.3.Postoperative nausea and vomiting and other adverse reactions.4.Postoperative hospital day.Results:There was no statistical difference in age,height,weight,and BMI in all three groups(P > 0.05).Compared with PCIA group,SPB group and SPB+B groupshortened extubation time and anesthesia recovery time(P <0.01).Compared with the PCIA group,the incidence of postoperative nausea and vomiting decreased in the SPB group and the SPB+B group(P < 0.05).Compared with the PCIA group and the SPB group,the resting and exercise VAS pain scores of the SPB+B group at 6h,12 h,24h decreased(P < 0.01).Compared with the PCIA group and SPB group,SPB+B group shortened the number of hospital stay(P <0.05).Conclusion Application of Ultrasound-guided SPB combined with brachial plexus block in modified radical mastectomy for breast cancer can not only not only has perfect postoperative analgesia,but also shorten the time of anesthesia recovery and extubation,reduce the incidence of postoperative nausea and vomiting,reduce the number of days in hospital.
