Objective:Missed abortion(MA)is a kind of pathological pregnancy and one of the special types of abortion.The cause of the disease has not been clear so far.It's high incidence seriously affects women's physical and mental health in China.In recent years,the importance of vitamin D as an immune agent at the maternal-fetal interface has been increasingly recognized.25-(OH)D has a long halflife and relative stability,which is the best index for the clinical detection of VD.The study tested the serum vitamin D content and serum immunological indicators IL-17(Interleukin 17,IL-17),IL-23(Interleukin 23,IL-23),IL-10(Interleukin 10,IL-10),TGF-?(Transforming Growth Factor?,TGF-?)in women with missed abortion and women who were required to terminate in normal early pregnancy.To study the expression levels of vitamin D and serum immunological indexes IL-17,IL-23,IL-10 and TGF-? in two groups of patients,and to explore the relationship between vitamin D and missed abortion from the perspective of immunology.To provide some theoretical support for the early prevention and treatment of missed abortion patients.Methods:Pregnant women who were randomly adimited to the Department of Obstetrics and Gynecology of the Affiliated Hospital of Yan'an University from 2018.5 to 2018.12 were randomly selected as the experimental group,who were diagnosed as having a miscarriage in accordance with the 8th edition of Obstetrics and Gynecology.At the same time,30 patients who were terminated in the Department of Obstetrics and Gynecology of the Affiliated Hospital of Yan'an University were randomly enrolled as a control group.According to the Endocrine Society Clinical Practice Guide,the total 25-(OH)D?20ng/mL in the serum of pregnant women is defined as vitamin D deficiency;20ng/?25-(OH)D?30ng/mL is vitamin D deficiency;25-(OH)D?30ng/ml is vitamin D sufficient.The experiment was reviewed and approved by the ethics committee of our hospital,and all patients signed an informed consent form.Peripheral venous blood was collected from all subjects without food after admission,the concentration of 25-(OH)D in serum was detected by Chemiluminescence method,and the serum concentrations of IL-17,IL-23,IL-10,and TGF-? were measured by Enzyme-Linked Immunosorbent Assay(ELISA).The obtained data were statistically analyzed using SPSS 22.0 software after statistical collation.Results:(1)The serum concentrations of IL-17((40.58 ± 14.15)pg/ml)and IL-23((31.25 ± 7.39)pg/ml)in the experimental group were higher than those((29.99 ± 12.22)pg/ml)and((25.45 ± 6.99)pg/ml)in the control group,and the differences were statistically significant(P = 0.003 < 0.05,P = 0.000 < 0.05);(2)The serum concentrations of IL-10((23.84 ± 3.58)pg/ml)and TGF-?((50.67 ± 5.82)pg/ml)in the case group were lower than those((39.39 ± 4.71)pg/ml)and((56.08 ± 6.71)pg/ml)in the control group,and the differences were statistically significant(P = 0.000,P = 0.01 < 0.05);(3)The concentration of serum 25-(OH)D in the case group was((15.67 ± 4.54)ng/ml)lower than that in the control group((24.87 ± 4.54)ng/ml),and the difference was statistically significant(P = 0.000 < 0.05).(4)In the experimental group and the control group,vitamin D was negatively correlated with IL-17(r =-0.719,p = 0.000 < 0.05;r =-0.752,p = 0.000 < 0.05)and IL-23(r=-0.642,P=0.000<0.05;r =-0.774,p = 0.000 < 0.05),IL-10(r = 0.435,p = 0.016 < 0.05;r = 471,p = 0.009 < 0.05)and TGF-?(r=0.571,P=0.01<0.05;r = 0.653,p = 0.000 < 0.05)showed a positive correlation.Conclusion:(1)The expression of IL-17,IL-23 is enhanced,and the expression of IL-10,TGF-? is decrease are related to the occurrence of missed abortion;(2)Vitamin D deficiency may be a high risk factor for missed abortion;(3)Vitamin D regulates the expression of IL-17,IL-23,IL-10 and TGF-?,which may be a potential mechanism to inhibit the occurrence of missed abortion.Vitamin D supplementation as early as possible may become an effective means of prevent missed abortion and maintaining a pregnancy.
