Survival Prognosis Of 100 Patients With Endometrial Cancer

Background:At present,the incidence of endometrial cancer is increasing year by year and showing a trend of rejuvenation.According to most reports,the 5-year overall survival rate of endometrial cancer is about 85%,and the overall prognosis is relatively good.However,some patients with endometrial cancer still die from postoperative cancer recurrence and metastasis.Therefore,this study retrospectively analyzed the clinical and follow-up data of patients with endometrial cancer in the Affiliated Hospital of Yan'an University to explore the prognosis and prognosis related factors affecting endometrial cancer patients,in order to screen out independent risk factors affecting prognosis;The clinical value of postoperative adjuvant radiotherapy and chemotherapy in improving the prognosis of patients with early endometrial cancer,in order to provide reference for the clinical diagnosis and treatment of patients with early endometrial cancer.Method:A retrospective study was performed on patients with endometrial cancer confirmed by postoperative pathology from January 2011 to October 2016 at Yan'an University Hospital.The complete clinical and pathological data of 100 patients who met the inclusion criteria were collected,including the age,menopausal status,vaginal bleeding,pathological grade,depth of myometrial invasion,pathological type,estrogen and progesterone receptor status,postoperative adjuvant treatment,Whether the lymph node dissection,vascular invasion and cervical involvement were performed,the risk factors were screened by Kaplan-Meier method,Log-rank test,P<0.05 was considered statistically significant,and Cox proportional hazard model was used for risk factors.Factor analysis,P <0.1 indicates that the difference is statistically significant;andthe postoperative adjuvant treatment of patients with early endometrial cancer is analyzed to explore its effect on the survival and prognosis of endometrial cancer.Result:Univariate Kaplan-Meier analysis showed that the survival rate of patients with endometrial cancer may be related to pathological type,pathological grade,estrogen and progesterone receptor,postoperative adjuvant therapy and vascular invasion(P<0.05).COX multivariate analysis showed that pathological type,pathological grade and postoperative adjuvant therapy were independent risk factors for prognosis in patients with endometrial cancer(P<0.1).Non-endometrial cancer patients had a higher risk of death than endometrioid adenocarcinoma.The patient's risk of death increased by 0.183 times compared with G1 and G2 patients,respectively,compared with G1 and G2 patients,respectively.The risk of death in patients undergoing adjuvant chemoradiotherapy was 0.130 and 0.194 times higher than that of postoperative observation and postoperative chemotherapy,respectively.Postoperative observation,postoperative chemotherapy and postoperative radiotherapy and chemotherapy in patients with stage I endometrial cancer had significant effects on survival outcome(P=0.007).Further stratification analysis revealed postoperative observation and postoperative radiochemotherapy for survival of patients.The prognosis had a significant effect(P=0.002).Postoperative chemotherapy and postoperative radiochemotherapy had significant effects on the survival prognosis of patients(P=0.047).Postoperative observation and postoperative chemotherapy had no significant effect on the survival prognosis of patients(P= 0.195).Postoperative chemotherapy in patients with stage I endometrial cancer <4 courses and ?4 courses had no significant effect on survival outcome(P=0.104).Conclusion:1.Pathological type,pathological grade,estrogen and progesterone receptor,postoperative adjuvant therapy and vascular invasion have an impact on the prognosis of patients with endometrial cancer.2.Pathological type,pathological grade and postoperative adjuvant therapy are independent risk factors for the survival and prognosis of endometrial cancer.The prognosis of patients with non-endometrial carcinoma,poor differentiation and postoperative radiotherapy and chemotherapy is poor.3.In patients with stage I endometrial cancer,the survival prognosis of postoperative observation and postoperative chemotherapy patients is better than that of postoperative radiotherapy and chemotherapy.4.Stage ? endometrial cancer after chemotherapy <4 courses and ? 4 courses have no significant effect on the survival of patients.
