| Objective:The changes of thromboelastogram(TEG)values in patients with long-term bed rest in neurosurgery were analyzed retrospectively, and the clinical early warning effect of thromboelastogram on deep venous thrombosis(DVT)of lower extremities was discussed.Methods:93 inpatients who were treated by neurosurgery in the 251 st Hospital of Chinese people’s Liberation Army from January to October 2017 were selected.According to the GCS score, the patients were divided into observation group and control group. In the observation group, the patients with consciousness disorder or limb dysfunction(GCS < 12)needed to stay in bed for a long time(bed stay > 21 days),while in the control group,it was clear that GCS > 12 was not needed for long term bed rest, and the patient was allowed to get out of bed as soon as possible.48 cases in the observation group and 45 cases in the control group were selected. The patients in the two groups were routinely treated with lower limb elastic socks, pneumatic therapy and physical prevention twice a day.All the patients in the observation group and the control group were examined for thromboelastogram in the morning of 1821 days after the first trimester of the first trimester, and the blood vessels of the lower extremities were examined by ultrasound on the day of each sampling. The TEG index value was recorded when the thrombus was found, and the abnormal value was significant. Results:There was significant difference between the observation group and the control group in the TEG index(MA value of blood clot maximum solidity,CI value of coagulation index). The mean value of MA in the observation group was significantly higher than that in the control group(P < 0.05),and the difference was statistically significant(P < 0.05).The mean value of CI in the observation group was significantly higher than that in the control group(P < 0.05).In the two groups, there were 7 cases of thrombosis in the observation group and 1 case in the control group under the premise of taking the relevant physical measures. The difference was statistically significant(P < 0.05).Conclusion:1.TEG has a certain value in evaluating deep vein thrombosis of lower extremities in patients with long-term bed-lying in neurosurgery.2.The MA value and CI value continued to increase continuously, high warning of thromboembolism. |
