Objective:Comparative observation and analysis of the catheter thrombolysis(catheter directed thrombolysis,CDT)and peripheral venous system thrombolysis in treating acute lower extremity deep vein thrombosis(the lower extremity deep venous thrombosis,LEDVT)in the early and mid-term clinical curative effect.Methods:Collecting the vascular surgery center of the fourth affiliated hospital of kunming medical university(the second people's hospital of vascular surgery center),the clinical data of patients with acute lower extremity deep vein thrombosis from October 2014 to September 2016.Combined with the patients who know the illness and according to economic conditions and willingness to patients,the catheter thrombolysis therapy of patients in group A(experimental group)are 89 cases,and patients with peripheral venous thrombolysis treatment system in group B(control group)are 106 cases.According to the clinical symptoms of patients before and after treatment,the change of limb skin tension,the change of limb week diameter and angiography image data,vascular patency rate and the incidence of PTS in 6 months or 12 months after discharge,assess both the early and mid-term clinical curative effect of thrombolysis treatment.Results:1.In the early clinical curative effect,Patients of two groups before and after thrombolytic therapy,the clinical symptoms such as skin tension were different degrees of improvement,side limb swelling has had fallen to ease.Detumescence rate of thigh and shank respectively are(84.98±19.65)%,(82.45±21.47)%in Group A.Detumescence rate of thigh and shank respectively are(66.35±24.40)%,(64.301±24.32)%in group B.the detumescence rate is similar between the two groups was statistically significant(p<0.05).The difference between the two groups was statistically significant(P<0.05)in the seven days of the course of the illness,and no statistically significant(P>0.05)in the 7 to 14 days of the disease.Treatment of three days,the group A is better than that of group B,the difference was statistically significant(p<0.05),treatment 7 days show the efficiency of two groups of patients was statistically significant(p<0.05),suggesting catheter can achieve faster and better clinical effect.The complication rate of group A was 5.6 percent,the complication rate of group B was 8.5 percent,and the complication rate of the two groups was not statistically significant(p>0.05).2.Acute LEDVT was observed in the clinical efficacy of the patients with the discharge follow-up after the discharge of the hospital,which was followed by A group of patients who were discharged from the hospital for 6 months,and the patency rate in group A was 75.9%.The patency rate in group B was 59.6%,and the two groups of patency rate were statistically significant(p<0.05).Patients in group A who were discharged from the hospital for 12 months were followed by an 11%incidence rate of PTS and 70.8%patency rate.Patients in group B of the incidence rate of PTS was 24.4%,and the patency rate was 53.7%.Two groups of vascular patency rate and the incidence rate of PTS had statistical significance(p<0.05),the group A medium-term curative effect is better than that of group B.Conclusions:For patients with acute lower extremity deep venous,The early curative effect of thrombolytic treatment was significant and the treatment of thrombolysis was better as early as possible.The use of the catheter is faster to obtain clinical efficacy,and accelerate the dissolution of the thrombus,and quickly open up the blockage.Patients who were discharged from the hospital for 6 or 12 months in group A were found to have a higher rate of patency rate and have a lower rate of PTS than group B.
