Objective This study was to analyze the clinical data of 406 patients undergoing prostate biopsy to investigate the correlation between neutrophil/lymphocyte ratio(NLR)and primary diagnosis of prostate cancer,so as to obtain a biochemical indicator that can predict prostate cancer diagnosis before prostate biopsy. Methods Retrospective analysis of 406 patients undergoing transrectal ultrasound-guided prostate biopsy from January 2015 to January 2018 in the Second Hospital of Tianjin Medical University, includes the patient's age, preoperative complete blood count(calculated NLR value),serum PSA,prostate volume, digital rectal examination(DRE),puncture pathology report and other clinical data. The Mann-Whitney U test and chi-square test were used to analyze the clinical characteristics and preoperative factors of patients, and to evaluate the relationship between neutrophil/lymphocyte ratio(NLR)and prostate cancer detection.Univariate and multivariate logistic regression analysis was used to assess the relationship between NLR and prostate cancer. All statistical data were analyzed by using SPSS statistcs 20 statistical software. Results 375 patients included in the study, including 183 patients with prostate cancer(PCa)and 192 patients with non-prostate cancer, were divided into PCa group and non-PCa group. In the two groups, the NLR of PCa patients was significantly higher than that of non-PCa patients(P=0.024),indicating that the preoperative NLR values were significantly different between PCa patients and non-PCa patients. The best NLR cut-off point value was 2.48 by analyzing the receiver operating characteristic(ROC)curve of the clinical data of 375 patients. All patients were divided into high NLR group(NLR ? 2.48)and low NLR group(NLR < 2.48)according to NLR cut-off point values. The comparison between the two groups showed that the high NLR group showed a significantly higher PCa detection rate than the low NLR group(P = 0.022).In addition, univariate and multivariate logistic regression analysis showed that NLR(HR 1.647,P = 0.032)and PSA(HR 1.096,P = 0.016)were independent risk factors for predicting prostate cancer. Conclusion Neutrophil/lymphocyte ratio(NLR)has a significant association with the risk of prostate cancer, and patients with high NLR values have a significantly increased risk of detecting prostate cancer.NLR is an independent risk factor for predicting prostate cancer.NLR can be used as an auxiliary biochemical indicator for predicting prostate cancer before prostate biopsy.
