Correlation Study Between Neutrophil Lymphocyte Ratio (NLR) And Malignancy Of Prostate Cancer In Preoperative Inflammatory Index

Objective:By collecting 174 cases of pathologically confirmed prostate cancer patients in our hospital,including the clinical and pathological data of TPSA,tumor invasion range,Gleason score,clinical stage,and bone metastasis.There were used to investigate the clinical value of preoperative neutrophil-to-lymphocyte ratio(NLR)for staging of prostate cancer.Methods:Obtained from October 2016 to October 2018 at the Yijishan Hospital affiliated to Wannan Medical College.Clinical data of 174 patients with prostate cancer who were hospitalized and confirmed by pathology.All patients were divided into groups according to the TPSA value,the Gleason score,the extent of tumor invasion,and whether bone metastasis occurred.The neutrophil and lymphocyte counts were collected,and the ratio between the two was calculated.The NLR value was obtained and expressed by mean±standard deviation.The ROC curve analysis was used to obtain the critical point of NLR evaluation.NLR?3.02 was used as the high NLR.NLR < 3.02 is a low NLR.Statistical analysis of NLR values in different groups was performed using SPSS 21.0software.Two independent sample t-tests were used for comparison between the two groups.Comparisons between groups were performed by one-way analysis of variance(ANOVA),and further by LSD method.Statistically significant indicators were compared in pairs.The classification and count data were expressed by the number of cases (percentage),and the comparison between groups was performed by the ?2 test.Spearman was used to analyze the correlation between NLR levels and various relevant indicators.The test level was P<0.05,which was statistically significant.Results:Referring to the inclusion exclusion criteria in this study,there were 174 patients with an age range of 45-80 years and an average age of 63.42 ± 10.06 years.The TPSA value was 4-10ng/ml: 23 cases,10-20ng/ml: 62 cases,20-50ng/ml: 71 cases,>50ng/ml:18 cases;85 cases of high-risk group,65 cases of middle-risk group for example,there were 24 patients in the low-risk group and 98 patients in the intracapsular group,only 76 patients in the capsule group and the infiltrating seminal vesicle group;34 patients with bone metastases.(1)The proportion of high NLR in patients in each TPSA group(8.7%,16.1%,26.8%,77.8%)showed an increasing trend(P<0.05);the NLR values of each TPSA group(2.00±0.64,2.25±0.89,2.70±0.84,3.79±0.93),the overall difference was statistically significant(P<0.05).The NLR levels between the two groups were analyzed by LSD method,and the differences were statistically significant(P<0.05).(2)The proportion of high NLR in high-risk group(37.6%)was significantly higher than that in middle-low risk(16.9%,8.3%)group(P<0.05);NLR value in high-risk group(2.93±0.92),middle-risk group NLR values(2.28±0.96),low-risk group NLR values(2.01±0.64),the overall difference was statistically significant(P<0.05).The differences between the two groups were analyzed by LSD method,and the differences were statistically significant(P<0.05).(3)The proportion of high NLR in the infiltrating seminal vesicle group was significantly higher than that in the restricted intracapsular group(13.3%)and only in the capsule group(28.3%)(P<0.05);NLR in the infiltrating seminal vesicle group The value(3.53±1.14)only broke the NLR value of the capsule group(2.80±0.88),and the NLR value of the intracapsular group(2.20±0.77).The overall difference was statistically significant(P<0.05).According to the LSD method,the differences were statistically significant(P<0.05).(4)The proportion of high NLR in patients with bone metastasis(55.9%)was significantly higher than that without bone metastasis(18.6%)(P<0.05).The NLR value(3.29±1.33)in patients with bone metastasis was significantly higher than that without bone.Patients with metastasis(2.38 ± 0.77)had statistically significant differences(P<0.05).(5)There was a significant positive correlation between NLR level and risk,extent of invasion and PSA level(rs>0,P<0.05).Conclusion:1.The greater the different TPSA values,the higher the Gleason score,the larger the tumor invasion range,the higher the NLR value,and the positive correlation between NLR and TPSA,the scope of invasion,and the Gleason score.2.The higher the NLR,the higher the malignancy of prostate cancer.High,the worse the prognosis will be,the NLR can be used as an indicator to provide reference value for the evaluation of PCa patients' condition.
