Objective To investigate the diagnostic value of bedside cardiopulmonary combined with ultrasound in children with pneumonia,and to evaluate the left and right heart function before and after treatment.Methods Twenty patients with clinically suspected pneumonia underwent chest CT and cardiopulmonary ultrasonography within 24 hours after admission.The results of LUS were compared with chest CT.The sensitivity and specificity of LUS in the diagnosis of children with pneumonia were analyzed with chest CT as the gold standard.Sex.After 7 days of treatment,cardiopulmonary combined with ultrasonography was performed to analyze the cardiac function and lung lesions before and after treatment.Results(1)50 children with clinically suspected pneumonia,chest CT diagnosis of 39 children with pneumonia;LUS diagnosis of 37 cases of pneumonia,sensitivity of 94.87%,specificity of 90.91%.(2)The main signs of pulmonary ultrasound within 24 hours of admission to children with pneumonia are local pleural line abnormalities(more than 5 mm of pleural line thickening,blurring,disconnection or disappearance),disappearance of the A line,alveolar-interstitial syndrome(3 lines or more in the lung field),lung consolidation and pleural effusion.After 7 days of treatment,the LUS signs were improved compared with those before treatment,that is,the sonogram showed the presence of the pleural line and the A line,the number of B lines decreased,and the consolidation range decreased or disappeared.(3)Echocardiography measured the parameters of left and right heart function.After clinical treatment,the Tei index decreased compared with before treatment,the difference was statistically significant(P< 0.05);E/e' decreased compared with before treatment,e' Compared with before treatment,the difference was not statistically significant(P >0.05);TAPSE was increased compared with before treatment,the difference was not statistically significant(P >0.05).Conclusions LUS has a high sensitivity and specificity in the diagnosis of childhood pneumonia.Cardiopulmonary combined with ultrasound can evaluate the changes of left and right heart function and lung lesions before and after treatment of children with pneumonia,and provide imaging evidence for clinical diagnosis and dynamic observation of clinical treatment effects.
