Objective:With the rapid progress of radiotherapy technology and the improvement of people's understanding of radiotherapy,the incidence of radiation pneumonia has increased significantly.So far,people's understanding of its causes and pathogenesis has not been clear.Glucocorticoids and antibiotics are often used in clinical treatment of radiation pneumonia.Long-term and large-dose use of glucocorticoids and antibiotics has obvious side effects,such as Cushing's syndrome,fatigue and femur.Bone necrosis and other symptoms.Therefore,it is particularly important to find effective and safe new drugs.Literature reports on the experimental and clinical studies of radiation pneumonia in traditional Chinese medicine,which can slow down the pneumonia manifestations of experimental animals and patients,improve the quality of life in the later period,and reduce the mortality due to radiation pneumonia.Mongolian medicine is rarely used in the treatment of radiation pneumonia,so it is a meaningful exploration to use Mongolian medicine to prevent and treat radiation pneumonia.Sachagu-3 Tangsan,as one of the classical ancient prescriptions of Mongolian medicine,is an effective ancient prescription for the prevention and treatment of pneumonia in clinic.There is no report of Mongolian medicine used to prevent and treat radiation pneumonia,so this drug was chosen as the subject to evaluate its therapeutic effect on rats.The theory of Mongolian medicine holds that the syndrome of radiation pneumonia is deficiency of lung-yin.Sachagu-3 Tangsan is a basic and effective ancient prescription for nourishing yin and clearing lung.The rat model of radiation pneumonia was established by comparing the lung coefficient,pathological changes of lung tissue,concentration of pro-inflammatory cytokines(TNF-alpha,IL-6)and pro-fibrotic inflammatory factors(TGF-beta 1)in plasma.Through the results of the study,the preventive and therapeutic effects of Sachagu-3Decoction powder on radiation pneumonia were compared,and its mechanism was explored actively,which provided reliable experimental basis for clinical patients with radiation pneumonia.Method:Eighty-one male rats of clean grade were randomly divided into: 1.blank control group;2.Sachagu-3 Decoction powder prevention group(Mongolian medicine prevention group);3.Sachagu-3 Decoction powder treatment group(Mongolian medicine treatment group);4.model group;5.dexamethasone treatment group(western medicine treatment group),of which 9 were blank control group and 18 were in other groups.6MV-X-ray linear accelerator was used to irradiate the whole chest of the experimental group at a single dose of 20 Gy,except for the blank control group.Sachagu-3 Tang powder was given to the Mongolian medicine prevention group 7days in advance,and the other groups were given intragastric administration on the second day after modeling.The dexamethasone treatment group was given intragastric administration of dexamethasone sodium injection mixed with normal saline.The blank control group and model group were given equal volume of physiological saline.The Mongolian medicine treatment group was given Sachagu-3Tang powder once a day.Three rats in the blank control group and six in the remaining group.Celiac anesthesia was performed with 5% chloral hydrate solution.The rats were killed at random on the 14 th,28th and 42 nd days.The rats' abdominal aorta blood was drawn around 4ml,placed in gel tube,centrifuged at 4 C 5000r/min for 10 minutes,plasma was separated and stored in-20 C refrigerator for preservation.The concentration of TNF-alpha,IL-6 and TCT-beta 1 in plasma of rats was measured by ELISA.The rats' weight and lung weight were weighed to calculate the lung coefficient.HE staining sections of the right lung lobe were taken and the differences between sections were observed under 10-fold light microscopy.Result:1)Comparisons of lung coefficient showed that Sachagu-3 Tangsan had better preventive effect than other groups except blank control group.There were significant differences in lung coefficient between the experimental groups(P < 0.05).It can be concluded that Sachagu-3 Decoction Powder can effectively reduce the lung coefficient of rats(P < 0.05).2)Comparing the pathological morphology of alveoli and pulmonary space(blood cell composition,fibrosis,inflammatory cells,etc.),it is concluded that the pathological morphological degree of the two experimental groups using Mongolian medicine is lighter than that of the model group.On the 14 th day,the exudation and infiltration of inflammatory cells in the dexamethasone group were lighter than those in the model group,but there was no significant difference in lung tissue between the two groups on the 28 th and 42 nd days.At each time point,the preventive pathological manifestation of Mongolian medicine was better than that of Mongolian medicine treatment group.In the dexamethasone treatment group,the lung abscess ranged from small to large and from small to large on day 28 and 42.The lung tissue structure of the blank control group was normal,and the expression was basically the same at each time point.3)Comparing the plasma levels of TNF-a,IL-6 and TCT-beta 1,it was found that the levels of TNF-a,IL-6 and TCT-beta 1 in the model group and the blank control group were significantly higher than those in the control group at all time points(P<0.05).The levels of TNF-a,IL-6 and TCT-beta-1 in plasma of rats in Mongolian medicine prevention group,Mongolian medicine treatment group and dexamethasone treatment group were significantly lower than those in model group(P < 0.05).The Mongolian pre-treatment group was lower than the Mongolian pre-treatment group and the dexamethasone treatment group(P < 0.05).The Mongolian group was lower than the dexamethasone group(P < 0.05).Conclusion:The Mongolian medicine has a preventive and therapeutic effect on radiation pneumonia.Possible ways of prevention and treatment are: 1.Increasing immunity and lung activity;2.Inhibiting the release of plasma pro-inflammatory factors TNF-alpha,IL-6 and pro-fibrotic cytokine TCT-beta 1 to protect the lung;3.Other unclear ways.From the experimental results,it is concluded that the drug has a certain effect on radiation pneumonia,and needs a large number of clinical trials to further verify.
