Prediction Of Premature Rupture Of Membranes By PCT, IL-6, Complement C3 And IgM | | Posted on:2020-05-04 | Degree:Master | Type:Thesis | | Country:China | Candidate:X Y Lin | Full Text:PDF | | GTID:2404330590987669 | Subject:Obstetrics and gynecology | | Abstract/Summary: | PDF Full Text Request | | Objective: By detecting the serum levels of procalcitonin,interleukin-6,complement C3 and immunoglobulin M in pregnant women,the correlation between the expression levels of procalcitonin,interleukin-6,complement C3 and immunoglobulin M was explored,in order to find more reliable indicators for Predicting Premature ruption of membranes,which could be used to guide clinical diagnosis and treatment,and avoid obstetric complications to the greatest extent.Methods: 121 pregnant women waiting for delivery in obstetrics department of our hospital from January to December 2018 were selected.Premature rupture of membranes did not occur in 65 cases(full-term premature rupture of membranes group);premature rupture of membranes occurred in 56 cases,including premature rupture of membranes in 14 cases(non-full-term premature rupture of membranes group),premature rupture of membranes in 42 cases(full-term premature rupture of membranes group).The serum levels of procalcitonin and interleukin-6 were detected by electrochemiluminescence,and the levels of complement C3 and immunoglobulin M were detected by immunoturbidimetry.Results(1)In the preterm premature rupture of membranes group,the proportion of procalcitonin greater than the standard value was 86.7%,interleukin-6 was57.1%,C3 was 0%,and immunoglobulin M was 7.1%.The number of cases with procalcitonin and interleukin-6 in preterm premature rupture of membranes was higher than the standard value,and the number of cases with preterm rupture of membranes was higher than that of preterm rupture of membranes and preterm rupture of membranes,and the number of cases with preterm rupture of membranes was higher than that of preterm premature rupture of membranes,with statistical significance(P < 0.There was no significant difference in the proportion of complement C3 and immunoglobulin M in each group(P >0.05).(2)Procalcitonin is associated with premature rupture of membranes.The higher the concentration,the more prone to premature rupture of membranes(the concentration of procalcitonin in the group of premature rupture of membranes is higher than that in the group of premature rupture of membranes at full term,and the difference is statistically significant)and the more prone to premature rupture of membranes at short term.Interleukin-6 is associated with premature rupture of membranes.The higher the concentration,the more prone to premature rupture of membranes(the concentration of interleukin-6 in the group ofpremature rupture of membranes before term is higher than that in the group of premature rupture of membranes at term,and the difference is statistically significant)and the more prone to premature rupture of membranes before term.There was no significant difference between complement C3 and immunoglobulin M in each group(p > 0.05).3.Premature rupture of membranes and premature rupture of membranes were combined into premature rupture of membranes group.The ROC curves of procalcitonin and interleukin-6 and combined predictive diagnosis of premature rupture of membranes were made.The area under the curve(AUC)was 0.677 and 0.622,0.731,respectively,with statistical significance P <0.05.The optimal cut-off value(procalcitonin > 0.064,IL-6 > 7.87)was the positive threshold(premature rupture of membranes).Calcium reduction was detected jointly.The sensitivity and specificity of procalcitonin and interleukin-6 were 89.64 and 76.15,respectively,which were significantly better than that of the two tests alone(P < 0.05),while the sensitivity and specificity of procalcitonin were 71.43 and 63.08,respectively,which were significantly better than that of interleukin-6(P < 0.05).Conclusions Pregnant women’s serum procalcitonin,interleukin-6 and their combined detection have certain predictive value for premature rupture of membranes.The levels of complement C3 and immunoglobulin M had no predictive value for premature rupture of membranes. | | Keywords/Search Tags: | Premature rupture of membranes, Procalcitonin, Interleukin-6, Complement C3, Immunoglobulin M | PDF Full Text Request | Related items |
