Premature rupture of membranes (PROM) is defined as the cause rupture that occurs prior to labour. It is a common complication during pregnancy and the incidence is 3.03—21.9%.If it occurs before the 37th week of pregnancy,it is called preterm PROM(PPROM). The incidence of PPROM is 3%, it is also related with 30-40% premature labor. PPROM,amniotic cavity and premature delivery are clinical syndrome which has a nearly correlation with infection. It is the familiar pregnancy complication that effects the incidence and the mortality of perinantic period mothers and fetuses. The most critical one of the factors inducing PROM is infection, and both sides affect each other. There are no clinical evidences of infection in most PROM patients with subclinical infections, though the diagnosis of chorioamnionitis is based on clinical signs, it is too late to treat the patients until the clinical symptoms and signs become obvious, and there is a serious complication to pregnant women, lying-in women and neonatal. Therefore, it is important for reducing the incidence of perinatic period mothers and fetuses that how to early diagnose PROM complicated chorion amniotis, and choose the most condign opportunity and the most appropriate way to terminate pregnancy.Recently the research shows that inflammatory cytokine maybe plays the important role of the cause of PPROM. The research discovers that they procure definite production to early diagnosis amniotic cavity infection by detecting the cell factors. This research is to detect and diagnose the extent of chorion amniotes and discuss the torecast value of chorion amniotes which is influenced by the level of IL-6,IL-8, TNF-αin amniotic fluid and serum of pregnant women by enzyme-united immunity adsorption, simultaneously detecting whether or not after delivery there is inflammation of placenta and caul of normal and PPROM pregnant women. Objectives:To evaluate the prediction by the levels of serum IL-6,IL-8,TNF—αin chorioamnionitis within pregnant after PPROM. Accordingly it provides the gist for the early diagnosis and therapy of chorion amniotes, especially provides the gist of the diagnosis and therapy for continuation of pregnancy therapy and antibiotic of complicating premature delivery. Methods:Choose 55 examples PPROM and 55 examples normal pregnant women as investigative objects. Detect the levels of IL-6,8,TNF—αin serum and amniotic fluid by ELISA. Examine and fetal membranes with microscope after delivery.Results:1. In amniotic fluid of PPROM pregnany:the level of. IL-6 is(133.8±35.7) pg/ml, IL-8 is (752.3±345.9)pg/ml,TNF-αis (100.4±45.2)pg/ml. The level is obviously higher than the control group. There is significant difference between two groups(P<0.05). The serum of PPROM pregnany:the level of IL-6 is (172.8±50.1) pg/ml, IL-8 is (376.7±131.8) pg/ml, TNF-αis (71.2±17.7) pg/ml. The level is obviously higher than the control group. There is significant difference between two groups(P<0.05).2. In PPROM group of the chorioamnionitis:the level of amniotic fruid IL-6, IL-8, TNF-αis (142.6±42.1) pg/ml and (905.3±363.5) pg/ml and (113.6±38.7)pg/ml; the level of serum IL-6, IL-8 is (189.3±39.2) pg/ml and (493.6±162.5)pg/ml. It is obviously higher than the patient of non-chorion amniotis. The difference of the two groups has the statistical significance(P<0.05).Conclusions:1,In PPROM group the level of serum and amniotic fruid IL-6, IL-8, TNF—αis obviously higher than the control group. The level of IL-6,IL-8, TNF-αis relate to PROM.2,In PPROM group of the chorioamnionitis the level of serum and amniotic fruid IL-6, IL-8, TNF-αis obviously higher than the patient of non-chorion amniotis. The level of serum IL-6, IL-8, TNF-αof the pregnant women is associated with the onset of PPROM and preterm labor caused by infection, it can be used for the index of reflection the early diagnosis of chorionamniotis.
