Objective 1.To study the concentration of sex hormones in BPH patients and compare it with young male outpatients;2.Including the index of prostatic inflammation IL-8,sex hormones and age,the risk factors of BPH were analyzed by multiple regression analysis;3.To study the prevalence rate of male delayed hypogonadism in BPH patients;4.To compare the concentration of sex hormones in LOH patients and non-LOH patients;5.Multiple regression the risk factors of the LOH patients.Methods Serum PSA,tPSA and transabdominal ultrasonography was used to measure the volume of prostate in patients with prostatic hypertrophy who accepted TURP or outpatients,and AMS questionnaire was filled out.The serum T,E2,PRL,P,LH and FSH was detected for Radioimmunoassay(RIA).Preoperative or outpatients was used to extract prostatic fluid by transrectal prostate massage.The T,E2 and PRL concentration were detected by RIA,and IL-8 concentration was detected by ELISA.The peripheral blood and prostatic fluid were collected from 20 outpatients of young male.The serum T,E2,PRL,P,LH,FSH concentration and prostatic fluid T,E2,PRL concentration were detected by RIA,and IL-8concentration in prostatic fluid was detected by ELISA.The peripheral blood and prostatic fluid were sent to the laboratory within 4 hours using a centrifuge for 10 minutes with3000r/min.The supernatant was taken and stored in a 2.0 ml freezing tube at-80? for unified detection.Grouping analysis:(1)According to whether prostatic hypertrophy,divided into two groups: BPH patients and young male,and the differences of serum T,E2,PRL,P,LH,FSH concentration,E2/T ratio and T,E2 and PRL concentration of prostatic fluid were compared between two groups;(2)According to the age of BPH patients,they were divided into two groups: 50-69 years old and 70-89 years old,and the serum T,E2,PRL,P,LH,FSH concentration and E2/T ratio were compared between the two groups.And the T,E2 and PRL concentration of prostatic fluid were compared;(3)Multivariate regression analysis was used to analyze the risk factors of BPH;(4)Study the prevalence rate of LOH in BPH patients by AMS score;(5)According to whether LOH in BPH patients,the levels of sex hormones in serum and prostatic fluid were compared with those to young male;(6)Multiple regression analysis was used to analyze the risk factors of LOH.Results 1.The serum T concentration of BPH patients was significantly lower than that of young male(p=0.000),The serum E2?LH?FSH concentrations?E2/T ratio of BPH patients was significantly higher than that of young male(p=0.002;p=0.026;p=0.009;p=0.000),there was no significant difference in serum PRL?P concentrations between BPH patients and young male(p=0.627;p=0.076);2.The Prostatic fluid T concentration of BPH patients was significantly higher than that of young male(p = 0.03),there was no significant difference in E2 ? PRLconcentrations of prostatic fluid between BPH patients and young male(p=0.187;p=0.543);3.The risk factors of BPH were age(p=0.008),prostatic fluid IL-8(p=0.000)and serum E2(p=0.027);4.LOH in the BPH patients with a prevalence rate of 85%,of which the mild,moderate and severe prevalence rates are: 35%,43%,7%;5.LOH patients with significantly lower serum T than non-LOH patients(z=-3.219,p=0.001)and young male(z=-6.187,p=0.000),serum P,LH were significantly higher than young male(p=0.037;p=0.024);non-LOH patients with significantly lower serum T than Young male(z = 0.001,p =0.000);6.LOH patients was significantly higher prostatic fluid E2 than non-LOH patients(z =-2.029,p = 0.042),LOH patients with significantly higher prostatic fluid T,E2 than young Male(p=0.000;p=0.039);7.Logistic regression analysis: The risk factor for LOH with BPH patients was: serum T decreased(p=0.009).Conclusion 1.The serum T,E2,LH,FSH concentration and E2/T ratio of BPH patients were significantly higher than that of young males.There was no significant difference in serum PRL and P concentrations compared with young males,suggesting that the decrease of T concentration and the increase of E2/T ratio and increase of LH and FSH concentration are related to the occurrence of BPH.2.The T concentration of prostatic fluid in BPH patients is significantly higher than that of young males.the E2 and PRL concentrations in prostatic fluid are not significantly different from young males,suggesting that elevated T concentration in prostatic fluid may be related to the pathogenesis of BPH;3.The risk factors of BPH include age,IL-8 in prostatic fluid and E2 in serum.4.the prevalence rate of LOH in BPH patients is85%,according to the severity of symptoms: mild,moderate,and severe prevalence rates are:35%,43%,7%;5.The serum T of LOH patients was significantly lower than non-LOH patients and young males,the serum P,LH was significantly higher than young males;the serum of non-LOH patients with significantly lower than young males,suggesting that thedecrease of T concentration and the increase of LH concentration are related to the occurrence of LOH;6.The prostatic fluid E2 in LOH patients was significantly higher than that in non-LOH patients.T and E2 were significantly higher than younger males,suggesting that elevated E2 in prostatic fluid may be related to the pathogenesis of LOH;7.The decrease of serum T concentration in BPH patients is an independent risk factor for LOH patients.
