Clinical Observation On Self-made Huoxue Jiedu Zhiyang Decoction For Treatment Of Uremic Pruritus

Objective:To observe the clinical effect of self-made Huoxue Jiedu Zhiyang decoction on the treatment of uremic pruritus,in order to relieve the pruritus symptoms of patients,improve the quality of life of patients,and provide new ideas for Chinese medicine treatment of uremic pruritus.Method:80 patients with pruritus from maintenance hemodialysis in our hospital from March 2017 to October 2018 were selected,and the subjects were divided into control group and treatment group by random number table method,40 patients in each group.Patients in the control group were treated with dialysis?hemodialysis+hemofiltration+hemoperfusion?and basic symptomatic treatment of western medicine;the treatment group was given Huoxue JieduZhiyang decoction on the basis of the control group.Two weeks was one course of treatment,and after two courses of clinical observation and intervention treatment,the efficacy of patients with pruritus was evaluated and relevant indicators were studied.Observation indicators include:skin itch score?visual analogue scale?and related biochemical indicators,details include:renal function indicators:blood urea nitrogen?BUN?,serum creatinine?Scr?,uric acid?UA?,?₂-microglobulin??₂-MG?;microinflammatory indicators:interleukin-6?IL-6?,tumor necrosis factor?TNF-a?;related indicators of calcium and phosphorus metabolism:parathyroid hormone?iPTH?,calcium?Ca?,phosphorus?P?;nutritional assessment indicators:hemoglobin?Hb?,albumin?ALB?.Results:There were no significant differences in gender,age,course of hemodialysis and all the observation indicators?score for itchy skin,BUN,Scr,UA,?₂-MG,IL-6,TNF-a,iPTH,Ca,P,Hb,ALB?between the treatment group and the control group before clinical intervention?P>0.05?;after 2 courses of intervention treatment:the total effective rate of the treatment group?90%? was significantly higher than the total effective rate of the control group?72.5%??P<0.05?;the score of pruritus,BUN,Scr,?₂-MG,IL-6,TNF-a,iPTH,P level in the treatment group was significantly lower than that in the control group?P<0.05?.There was no significant difference in UA,Ca,Hb and ALB between the two groups before and after treatment?P>0.05?.Conclusion:1.Huoxue jiedu zhiyang decoction can effectively improve the uremic patients skin itching symptoms,improve the quality of life of patients.2.Huoxue jiedu zhiyang decoction can improve a number of biochemical indicators in uraemic patients with pruritus.3.The mechanism of huoxue jiedu zhiyang decoction in the treatment of pruritus in uremic patients may be related to the protection of renal function,the improvement of microinflammatory state,and reduce parathyroid hormone,reduce blood phosphorus.
