Objective:(1)Understand the status of self-management,health locus of control,and illness perception in patients with chronic heart failure in Sanjia Hospital of Changsha City;(2)Analyze the status quo and influencing factors of self-management in patients with chronic heart failure,explore the relationship between self-management and health locus of control and illness perception,and provide new reference angles and ideas for self-management intervention in patients with chronic heart failure.Methods:(1)This study is a cross-sectional survey study,using a random sampling method,it is planned to select the first day of cardiovascular medicine in two tertiary hospitals in Changsha from May 2018 to October2018 and meet the exclusion criteria of 216 chronic heart forces.Depleted patients.Data were collected from patients using a general data questionnaire,a self-management,a health locus of control,and a illness perception(2)After collecting all the questionnaires,SPSS22.0 statistical software was used for data analysis: including descriptive statistics,t-test,analysis of variance,Spearson correlation analysis,and multiple linear stepwise regression analysis.Results:(1)The total score of self-management is(51.19±8.38),the score rate is 63.99%,and the scores of each dimension from high to low are: drug management(67.59%),diet management(67.45%),psychology andsociety.Adaptation management(67.36%),symptom management(58.95%).(2)The health internal control scores of patients with chronic heart failure were lower than the norm(23.65±3.85 vs 26.00±5.00,P<0.01),while the health authority and health opportunity scores were higher than the norm(24.63±2.84 vs 20.00±5.50;21.24±4.32 vs 15.00±6.00,P <0.01).Spearson correlation analysis showed that CHF patients' health internal control scores were positively correlated with self-management scores and drug,diet,psychological and social adjustment,and symptom management scores(r=0.335,0.219,0.161,0.321,0.292);Positive correlation with self-management scores and drug,diet,psychological and social adjustment,and symptom management scores(r=0.261,0.202,0.209,0.138,0.240);health opportunity scores and self-management scores and drug,psychological and social adaptation Symptom management scores were negatively correlated(r=-0.367,-0.400,-0.356,-0.217,P<0.01).(3)The illness perception score was(45.66± 6.04)points;Spearson correlation analysis showed that the total score of illness perception in CHF patients was negatively correlated with the total score of self-management and its diet,psychosocial adaptation,and symptom management(r=-0.246,-0.210,-0.218,-0.135,P < 0.05).(4)Univariate analysis of age,residence status,monthly income,medical payment method,education,course of disease,cardiac function classification,smoking history,and drinking history had statistically significant differences in self-administration scores(P<0.05).Multivariate linear stepwise regression analysis showed that monthly income,smoking,health control,health authority,health opportunities,cardiac function grading,disease course,and partner live into the regression model.The stepwise fitted multiple linear regression equationwas statistically significant(F=10.063,P<0.001).Conclusions:(1)Self-management of patients with chronic heart failure is generally at a medium level.(2)health locus of control of patients with chronic heart failure: Health internal control and health authority are positively correlated with self-management,and health opportunities are negatively correlated with self-management.(3)There is a negative correlation between the relationship between illness perception and self-management in patients with chronic heart failure.(4)Factors affecting self-management in patients with chronic heart failure include monthly income,smoking,health control,health control,health opportunities,cardiac function grading,disease duration,and partner residence.
