Objective: By studying the clinical characteristics of chronic heart failure(CHF)combined with non-thyroidal Illness Syndrome(NTIS),the differences between patients with different ejection fractions CHF combined with NTIS and the correlation between CHF risk factors and NTIS,the aim is to provide new ideas and new targets for the assessment and prognosis of CHF.Methods: Included 1138 patients from January 2017 to October 2020 in the first affiliated hospital of xinjiang medical university during hospitalization in patients with CHF,through our electronic medical record system,the research object of general clinical information collection,record the patient’s age,gender,nationality,smoking,alcohol consumption,blood pressure,BMI,etiology CHF,CHF complications,heart colour to exceed,NT-Pro BNP,we,routine blood,liver and kidney function,blood fat.Statistical analysis was performed.Results:(1)Compared with the CHF+ non-NTIS group,patients in the CHF+NTIS group were older(P < 0.001)and had a faster heart rate(P < 0.001);The proportion of patients with heart function grade IV was higher(P < 0.001);The levels of NT-Pro BNP(P < 0.001),left ventricular shumer pressure(P=0.004),serum creatinine(P < 0.001),homocysteine(P < 0.001),cystatin peptide C(P < 0.001)and C-reactive protein(P <0.001)were significantly increased.Ejection fraction(P < 0.001),proportion of obese patients(P < 0.001),systolic blood pressure(P=0.001)and diastolic blood pressure(P=0.011),hemoglobin(P < 0.001),total cholesterol(P < 0.001),high density lipoprotein(P < 0.001),low density lipoprotein(P < 0.001),serum sodium(P < 0.001)levels were significantly decreased.(2)HFPEF,HFMREF,and HFMREF.HFREF group accounted for 14.7%,28.4% and 56.9% of patients with low T3 syndrome,respectively,and the proportion of patients with low T3 syndrome was the highest in HFREF group,and the difference was statistically significant(P < 0.001).(3)Free Triiodothyronine(FT3)level was positively correlated with ejection fraction(r=0.093,P=0.002),albumin(r=0.378,P< 0.001),hemoglobin(r=0.354,P < 0.001)and BMI(r=0.247,P < 0.001).Inversely correlated with NT-Pro BNP(r=-0.460,P < 0.001),age(r=-0.257,P < 0.001),and creatinine level(r=-0.285,P < 0.001).Conclusion: Among the patients with different ejection fraction CHF,the proportion of HFr EF patients with NTIS was the highest.T3 level is inversely proportional to age and NT-Pro BNP level.The cardiac function and nutritional status of the patients with CHF complicated with NTIS are worse,and the anemia is more severe,so early intervention is needed. |
