Objective:Through the investigation on the prevalence of temporomandibular disorders among young men who took part in the physical examination of conscription of conscripts in Qingzhou City in 2015,2016 and 2017.The results were analyzed to determine the incidence of temporomandibular disorders in this group and to explore the etiology of temporomandibular disorder.The effect of two treatment methods was evaluated to provide reference suggestions for the physical examination of young candidates.Methods:The oral past history and temporomandibular joint function were collected from 2152 young men who participated in the physical examination of conscription of citizens in Qingzhou city.The data were sorted out and analyzed statistically to calculate the prevalence rate of temporomandibular disorder,the occurrence rate of each symptom and the distribution of clinical signs and past symptoms;193 of 407 patients diagnosed as temporomandibular disorders were randomly divided into two groups:group A and group B.And group A was given oral and maxillofacial system functional exercise therapy(Let the patient sit and relax the shoulders naturally.Look straight in front of eyes,swallowing achieved apical dislocation,and clench on 10 s,then relaxed the mandible at 10 s,slowly opening and closing mouth 10 times at constant speed from the postural position,keeping the chin point in the middle line when opening and closing the mouth,and limiting the opening degree of the jaw to35-40 mm.Closed mouth to mandibular position to a slight pause,and 4 times a day for functional exercise).The patients in B group were treated with total dental arch anatomic pad(maxillary impression was taken and superanhydrite model was perfused).The thick plastic sheet of 1.8mm was used as anatomic pad of maxillary complete dental arch by vacuum negative pressure molding mechanism.The patients were told to wear it all day for 3 months.After 3 months of treatment,the curative effect of TMD was evaluated according to the criteria of the American Association of Oral and Maxillofacial surgery(AAOMS).The cure rate and effective rate were calculated,followed up for 12 months,and the recurrence of the disease was observed.Results:In the study,2152 had different degrees of illness,with a prevalence rate of 18.91%.Of the 681 young people surveyed in 2015,145 cases had the disease and the prevalence rate was 21.3%,while among the 725 young people surveyed in 2016,there were 120 cases,with a prevalence rate of 16.6%.Among 746 young people surveyed in 2017,142 cases were ill with a prevalence rate of 19.0%,and there was no significant difference in the prevalence rate among the 746 young people in the past three years.Among the 2152 young conscripts,the highest incidence of bouncing was 17.7%.The incidence of mandibular dyskinesia was 8.3%,and that of pain was only 35(1.63%).Among the 407 patients with positive clinical signs,the incidence of sound alone was the highest(50.86%),followed by the sound associated with abnormal mandibular movement(36.85%),and the positive clinical signs accounted for 18.91%.And 18.91% of the total number of patients with positive clinical signs were 5.02%,50.86%,36.85% and 50.86%,respectively.After 3 months' treatment,all the symptoms of group A were improved after functional exercise treatment,and the effective rate was 100%.Among them,88 cases disappeared completely,the cure rate was 90.72%,and no recurrence was found in 12 months after follow-up.Of the 96 patients in group B,53 cases were cured,the cure rate was 55.21%,but 13 cases recurred after removal of the pad,and the recurrence rate was 24.52%.Conclusion:(1)The prevalence rate of TMD was 18.91% among young recruits in Qingzhou,but most of them were mild,and the symptoms were mainly sound.(2)The patients with temporomandibular disorders can be cured by the method of oral and maxillofacial system functional exercise,which should be preserved.
