Objective To analyze the relationship between magnetic resonance features of breast cancer and molecular markers ER,PR,HER-2,Ki67,molecular typing and biological behavior,to guide breast cancer patients to make clinical treatment plans,select and predict curative effect and prognosis,and to explore the value of MR examination in diagnosis and treatment of breast cancer.Method Through retrospective analysis of the preoperative MR imaging,pathological diagnosis and immunohistochemical results etc.of 59 breast cancer patients first diagnosed in Tangshan Maternal and Child Health Hospital from January 2017 to October 2018,the selected cases were all treated in hospital by operation.The pathological results after operation were the gold standard,and the immunohistochemical molecular markers ER,PR,HER-2,Ki67 were detected.To analyze the relationship between malignant signs(lobulation,burr,inhomogeneous enhancement)and various factors of breast cancer,including the relationship between malignant signs(lobulation,burr,inhomogeneous enhancement)and molecular typing,the relationship between malignant signs(lobulation,burr,inhomogeneous enhancement)and biological behavior of lymph node metastasis,the relationship between(lobulation,burr,inhomogeneous enhancement)and the expression of ER,PR,HER-2,Ki67 and the distribution trend of ER,PR from weak to strong.Result 1 The relationship between burr group and molecular typing of breast cancer: in the burr group,there were 11 cases of Luminal A,25 cases of Luminal B,6 cases of HER-2 over-expression and 1 case of tri-negative;in the non-burr group,there were 16 cases,2 cases of Luminal A,4 cases of Luminal B,4 cases of HER-2 over-expression and 6 cases of tri-negative.The expression rates of each molecular typing group were different among burr groups,with significant difference(P<0.05).2 The relationship between burr group and lymph node metastasis: 19 cases in burr group had metastasis,the metastasis rate was 44.19%,2 cases in non-burr group had metastasis,the metastasis rate was 12.50%.The metastasis rate was different among burr groups,the difference was statistically significant(P<0.05).3 The relationship between ER and burr group: There were 35 cases of ER positive expression in burr group,81.40% of ER positive expression rate,8 cases of ER negative expression,18.60% of ER negative expression rate;6 cases of ER positive expression in non-burr group,37.50% of ER positive expression rate,10 cases of ER negative expression and 62.50% of ER negative expression rate.There were significant differences in ER expression between the two groups(P<0.05).In the group with burr sign,there were 8 cases of ER(-),accounting for 18.60%;1 cases of ER(+),accounting for 2.33%;6 cases of ER(++),accounting for 13.95%;28 cases of ER(+++),accounting for 65.12%;In the non-burr group,there were 10 cases of ER(-),62.50%;0cases of ER(+),1 case of ER(++),accounting for 6.25%;5 cases of ER(+++),accounting for 31.25%.The expression rate of burrs was different from that of ER.Thedifference was statistically significant(P<0.05).4 The relationship between HER-2 and Ki67 in inhomogeneous enhancement group: in inhomogeneous enhancement group,there were 40 cases,of which 17 were HER-2 positive,with the positive rate 42.50%,23 were HER-2 negative,with the negative rate 57.50%;in homogeneous enhancement group,there were 19 cases,of which 2 were HER-2 positive,with the positive rate10.53%,17 were HER-2 negative,with the negative rate 89.47%.The positive and negative expression rates of HER-2 were different among the inhomogeneous enhancement groups,and the difference was statistically significant(P<0.05).In inhomogeneous enhancement group,there were 39 cases with high expression of Ki67,97.50% with high expression of Ki67,1 case with low expression of Ki67 and 2.50%with low expression of Ki67.There were 13 cases with high expression of Ki67,68.42%with high expression of Ki67,6 cases with low expression of Ki67 and 31.58% with low expression of Ki67 in homogeneous enhancement group.The high and low Ki67 expression rates were different among the inhomogeneous enhancement groups,and the difference was statistically significant(P<0.05).5 There was no significant difference between lobulation signs and molecular markers ER,PR,HER-2,Ki67,molecular typing and biological behavior(P>0.05).Conclusion 1 Luminal A and Luminal B were more common in molecular typing when burr sign appeared,and triple negative breast cancer was more common in non-burr sign.Chemotherapy schemes of different molecular typing were different.The ER expression rate in burr group was higher than that in non-burr group,and the trend of ER distribution from weak to strong was related to burr sign.The lower the burr positive expression rate,the weaker the ER expression.The higher the burr positive expression rate,the stronger ER expression.And the stronger ER expression,the better endocrine treatment effect or more suitable for endocrine therapy,I hope this will have a certain guiding role in the formulation of clinical chemotherapy program.Compared with non-burr group,burr group may be more prone to lymph node metastasis,and lymph node metastasis is also an important reference factor for clinical treatment.2 The positive expression rate of HER-2is higher in patients with inhomogeneous enhancement.Patients with inhomogeneous enhancement have higher probability of Ki67 high expression.Positive expression of HER-2 and high expression of Ki67 can predict high malignancy and poor prognosis of tumors,and can be used as reference factors for predicting prognosis.3 No direct relationship was found between lobulation and molecular markers ER,PR,HER-2,Ki67,molecular typing and biological behavior.Figure 6;Table 14;Reference 54.
