Objective: The purpose of this study is to retrospectively analyze the central lymph node metastasis of cN0 thyroid papillary microcarcinoma through meta-analysis.To explore the risk factors of central lymph node metastasis in cN0 papillary thyroid microcarcinoma,so as to provide theoretical basis for clinical treatment and metastasis risk assessment of cN0 papillary thyroid microcarcinoma and provide reference for clinical treatment. Methods: The databases of CNKI,Wanfang,Web of Science,Cochrane Library,PubMed,and springer were searched by computer.Collect the research on risk factors of central lymph node metastasis of cN0 papillary thyroid microcarcinoma published in the past 10 years at home and abroad, and meta-analysis was carried out on eligible inclusion documents. Results: A total of 14 studies were included, involving 5500 patients,of whom 1837 cases(33.4%)had central lymphatic metastasis. Results of Meta-analysis showed that male sex(OR:2.15,95% CI:1.86-2.48),young ag e(OR:1.53,95% CI:1.25-1.88),tumor size larger than 0.5 cm(OR:2.32,95% CI:1.83-2.93),bilateral(OR:1.51,95% CI:1.07-2.13),multifocal(OR:1.83,95% CI:1.36-2,46)and capsular invasion(OR:2.75,95% CI:2.02-3.75)were risk factors for central lymph node metastasis of cN0 papillary thyroid microcarcinoma(PTMC).Conclusion: cN0 papillary thyroid microcarcinoma patients with age <45years,male gender, tumor diameter≥0.5cm,bilateral foci, multiple foci or capsule invasion, should be vigilant of central lymphatic metastasis.
