Objective:In recent years,the incidence of thyroid papillary thyroid has increased globally.The proportion of papillary thyroid microcarcinoma has gradually increased,and its hotspots and controversies have become increasingly prominent.In the past 20 years,the incidence of PTMC is getting higher and higher,accounting for more than 90% of all new thyroid malignancies.PTMC is a common subtype of PTC,diameter ≤1.0cm,the survival rate is relatively high,but the local lymph node metastasis rate is higher.According to the data,PTMC has 5% cervical lymph node metastasis rate,2.5% distant metastasis rate,and cervical lymph node metastasis is a risk factor for recurrence and distant metastasis.PTMC clinical treatment is mainly for surgery,for the cleaning of the lymph node in neck central area,the medical community is more controversial.This study collected data from PTMC patients in the hospital from 2018.1-2018.12 and explored the risk factors for lymph node in neck central area.Methods:1.Case data of patients undergoing thyroid surgery concurrent with lymph node in neck central area dissection and obtaining pathological diagnosis of PTMC from 2018.1 to 2018.12 were collected.2.Retrospective analysis of 192 patients with complete medical records of PTMC patients with gender,age,maximum tumor diameter,capsule invasion and multi-focal,multiple regression analysis of risk factors for the metastasis of lymph node in neck central area.Results:1.From 2018.1 to 2018.12,192 patients with PTMC were admitted to the hospital.There were 54 patients with central metastasis,and the metastatic rate was 28.1%(54/192).2.192 patients with PTMC underwent lymph node in neck central area dissection.Univariate analysis revealed gender,age,maximum tumor diameter,capsule invasion,and multi-focal were associated with the metastasis of lymph node in neck central area(all P<0.05).All of the above factors were associated with the metastasis of lymph node in neck central area(P<0.05).Conclusion:This retrospective study concluded that the central lymph node metastasis rate of PTMC was 28.1%.Male,age(<45 years old),tumor diameter >0.5 cm,capsule invasion and multi-focal are risk factors for the metastasis of lymph node in neck central area.
