Background and objectiveMore and more thoracic surgeons have performed sublobectomy for patients with early-staged lung adenocarcinoma in recent years.There is still controversy in sublobar resection for invasive early-staged lung adenocarcinoma.The guidelines recommend sublobar resection should be only implcated for adenocarcinoma in situ or microinvasive adenocarcinoma.Therefore the accuracy of intraoperative frozen section is of importance.The study aimed to investigate the consistency of intraoperative frozen section and postoperative pathological examination and to evaluate the value of intraoperative frozen section in assessing the extent of early lung adenocarcinoma invasiveness.MethodsThe clinical data of 112 patients with early-staged lung adenocarcinoma were analyzed.The results between intraoperative frozen section and postoperative pathological examination were compered.ResultsThe overall diagnostic accuracy rate of intraoperative frozen section for early invasive lung adenocarcinoma was 86.6%(95/112),the diagnostic sensitivity was 86.2% and the differential rate for benign and malignant lesion was 98%,respectively.Multivariate analysis showed that the degree of invasiveness of early lung adenocarcinoma was only related to the size of the lesion(P=0.015).ConclusionIntraoperative frozen section can accurately detect the degree of invasiveness for most of early-staged lung adenocarcinoma.Thoracic surgeons need to look for other predictors of the degree of invasiveness to make resonable decisions during surgery.The size of the lesion is the main factor affecting the accuracy of the frozen section to assess the degree of invasiveness. |
