Objective:To investigate the characteristics of kinematics and plantar pressure distribution in patients with medial knee osteoarthritis(KOA)during walking. And it can provide objective data reference for conservative treatment such as insole and kneepad. Methods:We used Win-Track gait system to analysis the data of normal group(n=29)?mild KOA group(n=33)and severe KOA group(n=27).The data included the Western Ontario and McMaster Universities Osteoarthritis Index, the percentage of gait cycle at each gait phase, the time of gait cycle and the ratios of the average lateral peak pressure to the medial side in the forefoot?mid-foot?hind foot and fUll foot. Results:Compared the normal group with the mild group,we found that mild KOA group took more time(901.62ms±98.96ms)to complete the gait cycle. The results of foot forehead contact phase(FFCP)?foot flat phase(FFP)and foot forehead push-off phase(FFPOP)were no statistical differences between the two groups. The ratio of the average lateral peak pressure to the medial side in the hind foot of mild KOA group(0.95±0.10)was higher than the normal group(0.87±0.13).But the ratios of the average lateral peak pressure to the medial side in the forefoot?mid-foot?full foot were no statistical differences.Compared the normal group with the severe KOA group,we found that severe KOA group took more time(901.62ms±98.96ms)to complete the gait cycle.FFCP of severe KOA group(45.38%±17.81%)was higher than the normal group(31.76%±10.53%),but FFPOP of severe KOA group(20.23%±6.84%)was lower than the normal group(34.2607%±11.04%).The ratios of the average lateral peak pressure to the medial side of severe group in the forefoot?mid-foot?hind foot and full foot were higher than the normal group.Compared the mild KOA group with the severe KOA group,FFCP of severe KOA group(45.38%±17.81%)was higher than the mild KOA group(30.7g%±8.68%),but initial contact phase(ICP)and FFPOP of severe KOA group(9.97%±8.59%?20.23%±6.84%)were lower than the mild KOA group(14.60%±4.07%?31.46%±7.16%).The ratios of the average lateral peak pressure to the medial side of severe group in the forefoot?mid-foot and full foot were higher than the normal group, but the result of hind foot was no statistical difference. Conclusions:The patients with mild knee osteoarthritis do not change the pattern of walking and the distributions of plantar pressure only change locally. The patients with severe knee osteoarthritis change the patterm of walking significantly and the distributions of plantar pressure are biased to the lateral side.
