Objective:To analyze the risk factors and cognitive impairment in patients with cerebral microbleeds?CMBs?,and to explore the risk factors of cerebral microbleeds and its impact on cognitive function.Methods:According to the inclusion and exclusion criteria,93 cases of hospitalized patients with cerebral microhemorrhage in the department of neurology of the first affiliated hospital of henan university of science and technology from October 2017 to December 2018 were collected to balance the intergroup interference factors such as gender,age and education level.36 cases of patients with no abnormality in outpatient imaging examination?including SWI?in the same period were selected.Record patients'general information,the laboratory indexes and cerebral hemorrhage stove parts,number,diameter,application of Montreal cognitive assessment scale?MoCA?level of patients with cognitive analysis of two groups of patients with general information,the examination index and MoCA admission score,the single factor t test and chi-square test,the multi-factor Logistic regression analysis,single factor analysis of variance between groups and the Person/Spearman correlation analysis,analyze the related risk factors of brain micro bleeding and its effects on cognitive function.Results:1.Two groups of data and the check index compared commonly,CMBs group of patients with high blood pressure,neutrophil coun?NEU?t,neutrophils/lymphocyte ratio?NLR?,low density lipoprotein?LDL-C?,2 microglobulin decide to decide??MG?,fibrinogen?FIB?,c-reactive protein levels?hs-CRP?,homocysteine?HCY?,blood sedimentation?ESR?were significantly greater than the control group,but the platelet count?PLT?significantly lower than the control group,the difference was statistically significant?P<0.05?;Multivariate Logistic regression analysis showed that NLR?OR=1.372,95%ci:1.056-1.845?and hypertension?OR=3.512,95%ci:1.313-8.698?may be risk factors for CMBs?P<0.05?.2.The scores of MoCA,visuospatial executive ability,orientation ability,memory ability,naming ability,abstract ability and language fluency were compared between the two groups,and the differences were statistically significant?P<0.05?.3.The higher the number of bleeding sites,the higher the number grade,and the larger the diameter of CMBs,the lower the MoCA score was,and the higher of?MG,HCPR,HCY,and ESR in each examination index was related to the number of CMBs,while the higher of Lp-PLA2 was related to the CMBs diameter.4.Grouping according to different number levels of CMBs,MoCA scores and scores in various cognitive fields showed a decreasing trend as CMBs level increased,among which the differences between groups were statistically significant in executive function,naming,memory,language fluency,abstraction and MoCA total score?P<0.05?.With the increase of CMBs level,?MG,hs-CRP,HCY and ESR in all examination indexes showed an increasing trend,and the difference between groups was statistically significant?P<0.05?.5.According to the diameter of cerebral microhemorrhage,the larger the diameter of hemorrhage,the lower the MoCA score,and the difference was statistically significant in executive function and memory?P<0.05?.The lipoprotein-related phospholipase A2?Lp-PLA2?in each index was related to the diameter of microhemorrhage,and the difference was statistically significant?P<0.05?.6.For patients in CMBs group,Person and Spearman correlation analysis results showed that age,hs-CRP,HCY,glucose?GLU?,total bile acid?TBA?and FIB were negatively correlated with MoCA score?P<0.05?,while education level and CHE level were positively correlated with MoCA score?P<0.05?.Conclusions:1.Hypertension,NEU,NLR,LDL-C,?MG,FIB,hs-CRP,HCY,ESR and PLT were correlated with CMBs,while hypertension and NLR may be risk factors of CMBs.2.CMBs can lead to cognitive impairment in patients,mainly in visual space executive function,naming,memory,language fluency,abstraction and orientation.3.The higher the number of bleeding sites,the higher the number grade,and the larger the diameter of CMBs,the lower the MoCA score was,while the higher of the immune inflammatory indicators?MG,hs-CRP,HCY,and ESR were associated with the number of CMBs,while the higher of Lp-PLA2 was associated with the CMBs diameter.4.Increased age,hs-CRP,HCY,GLU,FIB and TBA may be risk factors for cognitive impairment in CMBs patients.
