Part ?:Analysis of related risk factors of cerebral microhemorrhage.Objective : Analysis of related risk factors of(CMBs)in patients with cerebral microhemorrhage Methods: A total of 240 patients with anterior circulation ischemic stroke hospitalized in the Department of Neurology of the affiliated Hospital of Inner Mongolia Medical University from December 2017 to December 2019 were collected.All the subjects are willing to cooperate with this study and accept the relevant experimental examination,and provide complete survey data at the same time.The patients were diagnosed as anterior circulation ischemic stroke by improving the head magnetic resonance T1 weighted image,T2 weighted image and diffusion weighted image(DWI).The head magnetic sensitivity weighted imaging(SWI)was performed to determine whether it was accompanied by CMBs,and improve the internal carotid artery ultrasonography.The two examinations were completed within 48 hours of admission,and the related tests were completed within 24 hours.Collect patients' sex,age,smoking history,diabetes history,heart disease history,systolic blood pressure(SBP),diastolic blood pressure(DBP),homocysteine(HCY),blood glucose,glycosylated hemoglobin,high density lipoprotein(HDL),cholesterol,triglyceride,low density lipoprotein(LDL),data collected by SPSS22.0 software for statistical analysis.Results:Among the 240 patients with anterior circulation ischemic stroke,there were 87 patients with CMBs and 153 patients with non-CMBs.The number of CMBs patients accounted for 36% of the total number of ischemic stroke patients.Among the related risk factors,the age of the CMBs group was 66.95 ±11.67,and that of the non-CMBs group was 63.07 ±7.36.There was a significant difference between the two groups(p< 0.05).CMBs group SBP: 167.24 ±26.69 mm Hg,DBP:92.66±13.53 mm Hg,non-CMBs group SBP: 151.12 ±24.75 mm Hg,DBP: 88.84 ±14.20 mm Hg,there was significant difference between the two groups(p < 0.05);CMBs group HCY:14.63 ±4.50 umol/L,non-CMBs group HCY:16.62 ±6.56 umol/L between the two groups;There was no significant difference in sex,smoking history,heart disease history,diabetes history,glycosylated hemoglobin,blood glucose,high density lipoprotein,cholesterol,triglyceride and low density lipoprotein between the two groups(p>0.05).(see Table 1)Among 240 patients,patients with hypertension and high HHcy were divided into simple hypertension group,simple HHcy group and H-type hypertension group.The results showed that CMBS were more likely to occur in simple hypertension group(x2 4.527,P: 0.033),simple HHcy group(x2 3.907,P:0.048),H-type hypertension group(X2 5.074,P: 0.024)(see Table 2).According to the echo property of plaque,240 patients with carotid atherosclerotic plaque were divided into three groups: low echo plaque group,isoechoic plaque group,hyperechoic plaque group and mixed echo plaque group.The results showed that there were 38 CMBS,37 non CMBS(x2.811,P: 0.002)in low echo plaque group,19 CMBS and 34 non CMBS(x2:0.005,P: 0.945)in isoechoic plaque group;in the hyperechoic group,there were 21 CMBS,41 non CMBS(x2 0.205,P: 0.651);in the mixed echo group,there were 21 CMBS,31 non CMBS(x2 0.491,P: 0.483)(see Table 3).The detection rates of CMBS were 43.68%,21.84%,24.14% and 24.14% in the low echo plaque group,isoechoic plaque group,hyperechoic plaque group and mixed echo plaque group,respectively,suggesting that the detection rates of CMBS in the low echo plaque group were significantly higher than those in the other echo plaque groups?Conclusion: With the decrease of DWI-Aspects score,the incidence of anterior circulation ischemic stroke with CMBs tends to increase,and whether there is a larger order of magnitude of CMBs in the low score group still needs further study.Part ?:Correlation Analysis between Cerebral microhemorrhage and Diffusion weighted Imaging ASPECTS scoreObjective:To analyze the relationship between the incidence of cerebral microhemorrhage and the ASPECTS score groups of diffusion weighted imaging.Methods:A total of 240 patients with ischemic stroke in the Department of Neurology,affiliated Hospital of Inner Mongolia Medical University from December 2017 to December 2019 were collected.After admission,the patients improved their cranial MRI+DWI+SWI imaging examination and were diagnosed as anterior circulation ischemic stroke,and were divided into two groups: CMBs group and non-CMBs group.DWI-Aspects scores were performed in the two groups.Results: Among the patients with anterior circulation ischemic stroke,52 had DWI-ASPECTS score less than 7,including 29 with CMBs and 23 with non-CMBs,and the incidence of CMBs was 55.8%.There was a significant difference between the two groups(p< 0.05).there were 23 patients with CMBs and 82 patients with non-CMBs,and the incidence of CMBs was significantly lower than that in other score groups(p< 0.05).Conclusion: With the decrease of DWI-Aspects score,the incidence of anterior circulation ischemic stroke with CMBs tends to increase,and whether there is a larger order of magnitude of CMBs in the low score group still needs further study.
