Relationship Between Brain Natriuretic Peptide Level And Depression In Patients With Acute ST Elevation Myocardial Infarction

Objectives:To investigate relationship between BNP levels and depression in the patient with acute ST elevation myocardial infarction.Methods:A total of 308 cases of patients in the department of cardiology of the affiliated hospital of chengde medical college with acute st-segment elevation myocardial infarction was enrolled in this study.They were divided into the group of BNP greater than 100pg/ml 124 cases and the group of BNP less than 100pg/ml 184 cases according to BNP level.The patients in the two groups were evaluated on the PHQ-9 scale within 72 h after admission,and the patients with depression were divided into mild depression,moderate depression,moderate and severe depression and severe depression according to their scores.The proportion of people without depression and the proportion of people with different degrees of depression in the two groups were analyzed.Results:1 Clinical data,biochemical indexes and the proportion of depression in the two groups were compared:1.1 Clinical data: Compared with the BNP<100pg/ml group,the BNP ?100pg/ml group was older [(64.66 12.51)vs(55.96 10.87),P<0.001];The proportion of women in BNP?100pg/ml group was higher [44%(54/124)vs.18%(33/184),P < 0.001];1.2 Biochemical indexes: Compared with the BNP<100pg/ml group,the Scr of the BNP?100pg/ml group was higher [70.63(61.61,91.06)vs.67.33(58.91,76.70),P=0.003].Urea nitrogen was higher in BNP?100pg/ml group [6.83(5.38,8.29)vs.5.60(5.18,6.96),P=0.001].EF value of BNP?100pg/ml group was lower [47.00(42.00,55.75)vs.55.00(48.00,58.00),P<0.001].1.3 Proportion of depression: compared with BNP<100pg/ml group,BNP?100pg/ml group had a higher proportion of depression [79.8%(99/124)vs.60.9%(112/184),P<0.001].2.Comparison of the proportion of patients with various depression grades between the two groups: 59 patients with mild depression(47.6%),28 patients with moderate depression(22.6%),10 patients with moderate and severe depression(8.0%),and 2 patients with severe depression(1.6%)in the BNP 100pg/ml group;There were 82 cases of mild depression(44.6%),23 cases of moderate depression(12.5%),6 cases of moderate and severe depression(3.3%),and 1 case of severe depression(0.5%)in BNP<100pg/ml group(P<0.001).The difference of depression degree between BNP?100pg/ml group and BNP<100pg/ml group was statistically significant.3.Spearman correlation analysis of BNP level and depression score in peripheral blood showed that BNP level was positively correlated with depression score(r = 0.45,P<0.001).4.Binary logistic regression analysis: After adjusting for confounding factors,BNP level?100pg/ml was an independent factor for depression(OR=2.09,95%CI: 1.19-3.67).Conclusions:The BNP level of patients with acute ST elevation myocardial infarction is closely related to depression.The higher the BNP level,the higher the proportion of people with depression,Depression was positively correlated with BNP level.
