Objective: Acute coronary syndrome(ACS) includes unstable angina(UA), non-ST-segment elevation myocardial infarction(NSTEMI) and ST-elevated myocardial infarction(STEMI), in clinical, a lot of researches about the ACS biochemical diagnosis markers have done. Based on this, this paper analyzes the impact of L-carnitine on ET-1 and brain natriuretic peptide level in elderly NSTEMI patients, and aims to provide a new theoretical basis for the diagnosis and treatment of NSTEMI.Methods: select 60 cases of patients whose age are over 60 and diagnosed with NSTEMI and admitted in the Geriatrics department and Cardiology department of Chengde Central Hospital 6 hours after paroxysm from Oct, 2013 – Aug, 2014 as the treatment group, and randomly divide them into L-carnitine group(A group) and conventional therapy group(B) groups. 6h, 12 h, 24 h, 3d, 7d, 14 d after admitted to hospital, all patients have to be collected with blood samples to measure the serum ET-1 and BNP levels. Select 30 cases of healthy elderly who come to Health Examination Center of Chengde Central Hospital for health examination and with the age over 60 as the control group. Extract with fasting blood samples to measure the serum ET-1 and BNP level. The ET-1 and BNP levels among the three groups were statistically analyzed.Results: comparison of serum BNP and ET-1 levels: the comparison among L-carnitine group, conventional therapy group and the control group, the serum BNP and ET-1 levels were significantly different at each time point(each P value <0.05), and the rand-size relationship is conventional therapy group > L-carnitine group >control group.The dynamic changes of serum BNP and ET-1 levels:(1) dynamic changes of serum BNP level: the serum BNP in conventional therapy group is bimodal secretion, the first peak value is higher than in L-carnitine group, occurrence time is12.50±0.70 h, occurrence time of the second peak is 156.60±7.34 h. While in L-carnitine group, there is only one peak value of serum BNP concentration, occurrence time is 15.84±0.98 h.(2) dynamic changes of serum ET-1: dynamic changes of ET-1 level in both L-carnitine group and conventional therapy group have a peak value, the conventional therapy group has a higher value with the occurrence time of 7.90±0.21 h, and the occurrence time in L-carnitine group is 16.70±1.30 h.Conclusions:1 The serum BNP and ET-1 levels in two treatment groups are higher than the control group, indicating that serum BNP and ET-1 levels increased during the evolution of the NSTEMI disease;2 Serum BNP and ET-1 levels in L-carnitine group are lower than conventional therapy group, indicating that L-carnitine can decrease the increasing degree of serum BNP and ET-1 in NSTEMI patients3 The change of serum BNP and ET-1 levels indicating that the application of L-carnitine therapy can decrease the ET-1 and BNP levels in NSTEMI patients, indicating the inhibiting effect of L-carnitine on neuroendocrine activation after occurrence of NSTEMI, which is conducive to patients recovery.
