Objectives 1.To investigate the status quo of uncertainty in illness in patients with primary glaucoma,analyze the main manifestations of patients' uncertainty in illness,and formulate a more targeted information support program based on the results of the survey.2.To Apply targeted information support to clinical practice,and discuss the impact of targeted information support on uncertainty in illness of primary glaucoma patients.Methods The study was divided into two phases.One was a survey study,83 in-patients with primary glaucoma who met the exclusion criteria were selected in the study by convenience sampling.The general data questionnaire and Mishel's uncertainty in illness scale-adult version were used to investigate the patients.The level of uncertainty in illness and its influence factors were analyzed.The second is the intervention research.Based on the results of previous investigations,with reference to relevant literature,to improve the information support content for patients with primary glaucoma.72 patients with primary glaucoma who met the inclusion and exclusion criteria were enrolled as the subjects.According to the random number table method,the patients were divided into the experimental group and the control group with 36 cases each.During hospitalization,the control group was given routine health guidance,and the experimental group provided the patient with targeted information support based on the previous investigation on the basis of routine health guidance.The Chinese version of Mishel's uncertainty in illness scale-adult was used to assess the uncertainty in illness on the day of hospitalization and on the day of discharge.Result 1.The investigation research: A total of 78 patients completed the survey,and the sample recovery rate was about 94.0%.The average total score of uncertainty in illness in patients with primary glaucoma was 105.53±8.16,which was in the middle level.The order of the average score of items in each dimension from high to low was ambiguity dimension(3.65±0.28),unpredictability dimension(3.57±0.41),inconsistency dimension(3.13±0.44)and complexity dimension(2.62±0.34).Univariate analysis showed that there were significant differences in the scores of uncertainty in illness among patients with primary glaucoma among different ages,education levels,marital status,monthly household income per capita,type of medical insurance,number of hospitalizations,and duration of disease(P<0.05).The older the age/the lower the education level/the lower the monthly household income per capita/the first hospitalization or the shorter the disease course,the higher the score of uncertainty in illness among the patients;in the marital situation,the widowed patients had the highest uncertainty score.Further analysis showed that the degree of education,marital status and the course of disease entered its regression equation.2.The intervention research: A total of 69 patients completed the intervention study,including 35 in the experimental group and 34 in the control group.Before the intervention,there was no significant difference in the general data and the scores of uncertainty in illness between the experimental group and the control group(P>0.05).After the information support intervention for primary glaucoma patients,the average total score of uncertainty in the primary glaucoma of the experimental group was 93.77±5.52,and the average score of the patients before intervention was 108.26±4.69,the difference was statistically significant(P<0.05).In the scores of each dimension,there were statistically significant differences in the experimental group after intervention(P<0.05).In the control group,the average score of uncertainty in illness before intervention was 109±4.01,which was statistically significant compared with post intervention(P<0.05),and the scores of ambiguity dimension,complexity dimension and unpredictability dimension were significantly significant after intervention(P< 0.05),but in the unpredictable dimension,there was no significant difference in the control group after intervention(P>0.05).The comparison between the two groups of patients with uncertainty in illness showed that the average score of the control group after the intervention was 101±6.72,and the average total score of the control group was higher than that of the experimental group(P<0.05);there were significant differences in the scores of each dimension of the uncertainty in illness in the two groups,the ambiguity dimension(P<0.05),complexity dimension(P<0.05),inconsistency dimension(P<0.05),unpredictable dimension(P<0.05).Conclusion 1.The level of uncertainty in illness in patients with primary glaucoma was at a medium level,the main source of which is ambiguity,followed by unpredictability.Medical staff should provide patients with relevant knowledge about the clinical manifestations of the disease,its impact on life and the progress and prognosis of the disease,so as to enhance the patient's perception of primary glaucoma.2.Clinically,when providing information support for patients,medical staff should not only communicate in a plain and easy-to-understand language,but also pay attention to the information provided by patients with shorter course of disease to meet the information needs and alleviate the uncertainty in illness.3.According to the information content determined on the basis of previous investigation research,targeted information support for patients,is beneficial to reduce the uncertainty in illness in patients with primary glaucoma,and enrich the clinical nursing pathway for clinical nurses to provide nursing services for patients with primary glaucoma.
