Objective:Benign paroxysmal positional vertigo(BPPV),also known as otolith,is the most common form of clinical peripheral vertigo.Usually when the head moves to a specific position,such as lying down,sitting up,looking up,bending over,turning quickly,or getting up when jumping up,more often with horizontal tangible nystagmus and nausea Autonomic symptoms such as vomiting,which lasts for a short period of time,usually within one minute.The main method of treatment is the otolith reduction method,which can be completed by a doctor by hand or by using a corresponding treatment instrument,and the clinical effect is good.In the clinical treatment of long-term patients,the research team found that some patients had obvious improvement in vertigo after treatment with otolith reduction,and the nystagmus disappeared,but there were still dizziness,top-heavy,cotton-like feeling,or floating.Residual vertigo symptoms such as sensation,unstable walking and vibrating illusion,so consider whether the otolith reduction method can be combined with traditional Chinese medicine to treat BPPV.We believe that dizziness is caused by clearing and clearing,and is closely related to liver,spleen and kidney.Ginkgo biloba leaves are neutral in Chinese medicine,sweet and bitter,and enter the heart and lungs.They have the effect of promoting blood circulation and dispersing phlegm,relieving pain from the collaterals,concentrating lungs and relieving asthma,and reducing fat and turbidity.And modern pharmacological studies show that Ginkgo biloba extract not only regulates hemodynamics,but also regulates the rheological state of blood,and can also eliminate excess reactive oxygen free radicals in the human body,thereby improving the inner ear.The circulatory disorder.Ginkgo ketone ester is a fifth-generation ginkgo leaf preparation developed by China,namely Ginkgo biloba extract.It has high active ingredient and low sensitizing ingredients and is widely accepted at home and abroad.Therefore,in this study,patients with BPPV were treated with otolith reduction combined with ginkgo ketone ester dropping pills and otolith reduction combined with betahistine mesylate to compare the improvement of dizziness symptoms in the two groups to explore otolithic reduction.The method combines the therapeutic effect of ginkgo ketone ester dropping pills on BPPV,and thus is popularized for clinical use.Method:72 patients who were diagnosed with BPPV from the neurology department of the Affiliated Hospital of Chengdu University of Traditional Chinese Medicine or the inpatient department were selected to be randomly divided into two groups,A and B,with 36 cases in each group.,A is the test group,and B is the control group.The two groups were treated with otolith reduction combined with ginkgo ketone ester dropping pills(Ginkgo Bismuth Shanxi Qianhui Pharmaceutical Co.,Ltd.),otolith reduction method combined with betahistine mesylate tablets(Minshilang Essence China Pharmaceutical Co.,Ltd.)treatment of patients with BPPV.After collecting the general data of the patients,the patients were followed up for 7 months in the course of treatment,and the difference was analyzed according to the DHI and vertigo VAS scores before and after treatment,and the final therapeutic effect was achieved by nimodipine method.It is determined that the data is managed and analyzed using SPSS 22.0 software.The measurement data is expressed as meanąstandard deviation.For data obeying normal distribution and homogeneity of variance,t test is used,and rank sum test is used for data that does not obey normal distribution.The count data is expressed by the frequency,using the X~2 test;the measurement data is repeated several times,and the data is analyzed using the repeated measurement variance.According to the final measurement results,the clinical effect of otolith reduction combined with ginkgo ketone drop pills in the treatment of patients with BPPV was determined and evaluated.Result:1.After the end of the experiment,the DHI score and the vertigo VAS score of the two groups were statistically significant compared with the pre-treatment data.2.At the end of the experiment,the DHI score and VAS scores of the first,third,fifth,and seventh days of treatment were statistically significant between the experimental group and the control group,and the control group was superior to the experimental group.There was no statistically significant difference between the two groups in the first month,March,and June follow-up;3.After the end of the experiment,the recurrence rate of the two groups was compared.The recurrence rate was 0%in the experimental group and 20%in the control group.The difference between the two groups was statistically significant(p=0.028).4.Comparison of adverse reactions.After the end of the experiment,the data of the two groups were analyzed.The proportion of adverse reactions in the experimental group and the control group were 0%and 6.67%,respectively.After comparison,there was no statistically significant difference between the two groups(p=0.238).Conclusion:1.Otolith reduction combined with ginkgo ketone ester dropping pills and otolith reduction combined with betahistine mesylate tablets can improve the symptoms of vertigo in patients with BPPV,which deserves further study.2,otolith reduction combined with betahistine can improve vertigo symptoms more quickly and effectively,but in the total treatment process,otolith reduction combined with betahistine did not show better therapeutic effect,indicating two The drug has the same effect in improving the symptoms of vertigo,and there is no significant difference.3.After the end of the experiment,the recurrence rate of the two groups was compared.The difference between the two groups was statistically significant(p=0.028).From the specific values,the recurrence rate of the control group was higher than that of the experimental group.The otolith reduction combined with ginkgo ketone ester dropping pills showed a lower recurrence rate in the treatment of BPPV.4.The rate of adverse reactions of otolith reduction combined with ginkgo ketone ester dropping pills group was 0%,and the adverse reaction rate of otolith reduction combined with betahistine mesylate was 6.67%.There was no significant difference in adverse reaction rates between the two groups(p=0.238).
