Objective: This research aims to determine the risk factors of colorectal polyps by studying the relationship between metabolic syndrome, its components, related metabolic factors and colorectal polyps,so as to provide important reference and guidance for prevention of colorectal polyps.Method: 200 cases of colorectal polyps that met the requirements and 175 control group cases that showed no abnormality in colonoscopy were selected from the inpatients who underwent colonoscopy in the enteroscopy room of gastroenterology department of qinhuangdao first hospital during July 1st,2017 to December 31 st,2018.According to the pathological type, the colorectal polyp group was divided into the colorectal adenomatous polyp(CAP)group of 147 cases, the colorectal non-adenomatous polyp(NCAP)group of 53 cases.According to the polyp distribution, the colorectal polyp group was divided into the right half colon group of 45 cases, the left half colon and rectum group of 69 cases, and the whole colon group of 86 cases.Data such as gender, age, height, weight, medical history information were collected, and blood samples are collected for examination to get venous fastingd glucose, serum triglyceride, serum cholesterol, serum high-density lipoprotein cholesterol, serum low density lipoprotein cholesterol and blood uric acid levels, the pathologic results of the polyps and their performance under colonoscopy(polyp size, number, distribution)were recorded. The above clinical data were collated, entryed into Excel sheets, then analyzed by SPSS 17.0 statistical software.Result:1.There are no significant differences in gender and age between the case group and the control group(P>0.05).2.In the patients of case group, the detection rate of metabolic syndrome is 20%,the detection rate of obesity is 16%,and the prevalence of type 2 diabetes is 17.5%.The detection rate of metabolic syndrome, obesity and type 2 diabetes in the case group are higher than that in the control group(P<0.05).The body mass index(BMI),fasting-blood glucose and triglyceride levels in the case group are all higher than those in the control group(P<0.05).3.The detection rate of metabolic syndrome is 23.81% in patients of the CAP group,9.43% in patients of the NCAP group, and the detection rate of metabolic syndrome in CAP group is higher than that in NCAP group(P<0.05).The detection rate of obesity is 12.93% in patients of the CAP group,24.53% in patients of the NCAP group, and the detection rate of metabolic syndrome in the NCAP group is higher than that in the CAP group(P<0.05).The average BMI of NCAP group is higher than that of CAP group(P<0.05).4.The detection rate of metabolic syndrome is 26.74% in patients of the whole colon group,6.67% in patients of the right half colon group, and the detection rate of metabolic syndrome in the whole colon group is higher than that in the right half colon group(P=0.006).The average BMI of the whole colon group is higher than that of the right half colon group(P=0.002).5.BMI is positively correlated with the number of polyps(r=0.158,P<0.05);BMI is negatively correlated with the pathological grade(r=-0.145,P<0.05);BMI is negatively correlated with the size of polyps(r=-0.092,P<0.05);fasting-blood glucose is positively correlated with the number of polyps(r=0.217,P<0.05).6.BMI,fasting-blood glucose and triglyceride are risk factors for the occurrence of colorectal polyps, and they were positively correlated, the OR value are 1.077,1.185 and 1.750 respectively.Obesity is a protective factor for the occurrence of colorectal adenomatous polyps and they are negatively correlated, with an OR value of 0.449.Conclusion:1.Metabolic syndrome is the influencing factor for the occurrence of colorectal colorectal polyps and adenomatous polyps.2.Obesity,type 2 diabetes,BMI,fasting-blood glucose and triglycerides are the influencing factors for the occurrence of colorectal polyps.3.Polyps are more likely to occur in the whole colon in patients with metabolic syndrome and high BMI.4.BMI and fasting-blood glucose are positively correlated with the number of polyps.5.BMI,fasting blood glucose and triglyceride are risk factors of colorectal polyps.
