Objective: This article is aiming at inhancing the recognition a kind of disease-tracheal adenoid cystic carcinoma.We select a case report to demonstrate the epidemiology,pathogenesis,clinical presentation,diagnosis,treatment and prognosis of the tracheal adenoid cystic carcinoma.Method: Collect the date of are patient who is diagnosed as TACC and treated in the First Department of Respiratory Medicine of the Third Hospital of Hebei Medical University(our hospital).discuss the epidemiology,pathogenesis,clinical presentation,diagnosis,treatment and prognosis of the tracheal adenoid cystic carcinoma and review related articles.Results: The patient is a 65-year-old man with the complains of cough,sputum and chest distress for 20 days.He is transfered to department of respiration in order to receive the examination of bronchoscope.Chest CT demonstrates cramped trachea.The cauliflower-like mass under bronchoscope is confirmed to be tracheal adenoid cystic carcinoma.This patient receive several electrocoagulation and electrocautery treatment through bronchoscope.Radiation(total dose GTY 66 GY CTV1 60 GY,single dose 2.54 GY CTV1 2.3GY,treatment times 26 times + total dose GTY 15.2GY,single dose 2.54 GY,treatment times 6 times)and chemotherapy(CAP regimen,cyclophosphamide 0.8g d1,8,adriamycin 60 mg d2,cisplatin 30 mg d3-5,treatment times 2 times).Conclutions:1.Tracheal adenoid cystic carcinoma(TACC)is a rare low-grade malignant tumor with insidious onset,long course of disease,and the specific pathogenesis is unclear.2.The clinical symptoms are related to airway obstruction.There are no symptoms in the early stage,and the main manifestations in the middle and late stage are cough,wheezing,ect.It's easy to be missed and misdiagnosed.Although it is a low-grade malignant tumor,it still has the potential of malignant metastasis.3.Electronic bronchoscopy is the most direct and effective method for diagnosing tracheal tumors,and accurate diagnosis depends on pathology.4.Surgery is the preferred treatment method,which can significantly improve the survival rate of patients.Bronchoscope intervention can rapidly and effectively relieve the symptoms of dyspnea of patients,which creating the possibility for further treatment.Postoperative or endoscopic interventional therapy combined with radiotherapy can also achieve long-term Survival with a tumor.5.The clinical application and research of targeted drugs are rare.It's further research and exploration will provide potential new therapies for solving chemotherapy-resistant cancer and bring new hope for TACC patients.
