| ObjectPrimary breast adenoid cystic carcinoma(Adenoid Cystic Carcinoma,ACC)is a special histological types of breast cancer,the incidence rate of ACC is less than 0.1% of breast cancer,but with the incidence rate of malignant tumors increasing in recent years,the incidence of ACC also tends to increase.ACC phenotype is often triple negative,but unlike the high recurrence、high transfer and high mortality clinical outcomes of triple-negative breast cancer,ACC prognosis is pretty good.They use a conservative treatment: breast-conserving surgery or simple mastectomy abroad,but currently its treatment regimens are inconsistent in domestic,papers about ACC is also fewer,we selected 26 cases of primary breast ACC in sixteen years,made a summarize of their clinicopathological features and reported with literature review disussion to provide the basis for clinical treatment choice of breast ACC in domestic.MethodSelect 26 cases admitted to the Tianjin Medical University Cancer Hospital Breast Center and was pathological diagnosed as primary breast ACC from June 1996 to August 2013,we summarized the clinical-pathological data、prognosis information and statistical analysis of 26 patients,we also chose one case to do the p63、CK、SMA immunohistochemistry staining。 Result26 cases of breast ACC patients(25 females and one male),median age 54 years(39 to 79 years),The average maximum tumor diameter is 2.4cm(1.2 ~ 9.5 cm).Histopathological features showed nests are constituted by the true glandular and pseudolumina,cells of true glandular are CK,EMA positive and cells of pseudoglandular are myoepithelial markers,IV collagen positive.The molecular type of most ACC is triple negative(do not express ER,PR,HER-2).26 cases of ACC had no lymph node metastases.The median follow-up time of 55 months(3 to 137 months),no recurrence,distant metastasis and death happened,the prognosis is pretty good.ConclusionPrimary breast ACC is a special histological type with a good prognosis of triple negative breast cancer,conserving surgery and / or mastectomy treatment usually without lymph node dissection are the most common treatment in developed countries,clinical management should be distinguished from other histological type of triple negative breast cancer. |
