Objective:Perioperative hypothermia is clinically common and can cause a series of complications.The purpose of this study was to observe the incidence of hypothermia during unilateral hip replacement in elderly patients with general anesthesia,and evaluate the clinical effect of thermal reflective blanket and intravenous fluid warming.Methods:During August 2018-March 2019,40 cases of elderly patients undergoing elective hip replacement,ASA sizing for? or ?,aged between 60-80,27 males and 13 females,were randomly divided into two groups?n=20?:control group?group A? and warm group?group B?.Two groups of patients were given conventional heat preservation nursing,the group B patients in the premise of conventional heat preservation nursing use heating blanket and heating infusion device.The operating room temperature was controled at 22??24?,The basal fluid volume of the two groups was 6?8ml·kg-1·h-1,and the crystal-gel ratio was 2:1.Intraoperative monitoring of patients' nasopharyngeal temperature at the time points:including entering the operating room?T0?,immediately after anesthesia intubation?T1?,the beginning of the operation?T2?,every 15min during the operation?T3,T4,T5?,the end of the operation?T6? and leaving the operating room?T7?.Record the amount of intraoperative blood loss and observe whether arrhythmias occurred during the operation and whether shriving occurred after the patients enter into the anesthesia recovery room,and assess the patient's thermal comfort score.Results:The comparison of nasopharyngeal temperature at various points in group A shows that there was no significant difference when T1 compared with T0,P>0.05,and when T2?T7 compared with T0 the difference was statistically significant,P<0.05.There was no statistical difference when T1?T7 compared with T0 in group B,P>0.05.There was no statistical difference between the two groups'nasopharyngeal temperature at T0,T1?P>0.05?.The nasopharyngeal temperature in group B was significantly higher than that in group A at the time of T2?T7?P<0.05?.The incidence of hypothermia in group B?20.0%? was significantly lower than that in group A?80.0%?,the difference was statistically significant?P<0.05?.There was no statistical difference between the intraoperative blood loss of the two groups and the same to the shivering?P>0.05?.Thermal comfort score of group B was significantly higher than that of group A?P<0.05?.No arrhythmia occurred in the two groups.Conclusion:The incidence of hypothermia in group B was significantly lower than that in group A,indicating that the heat preservation measures adopted in the elderly hip replacement surgery under general anesthesia can effectively prevent the decrease of nasopharyngeal temperature and improve the comfort level of patients.
