The Comparison Of Stress Responses During Elderly Hip Replacement Between Unilateral Lumbar Hemp And General Anesthesia

Objective:To explore the effect in elderly hip replacement, observe the comparison of stress responses during elderly hip replacement between the unilateral lumbar hemp using hypobaric bupivacaine and general anesthesia.Methods:Forty elderly patients undergoing elderly hip replacement were randomly divided into two groups with 20 each, unilateral lumbar hemp (group Ⅰ) and general anesthesia group (group Ⅱ).Venous blood were collected to determinate the concentration of blood sugar, serum catecholamine (CA) and cortisol (CORT) at 10min before anesthesia (To), end of the surgery (T1) and 24h after the surgery (T2).BP, HR, pulse O2 saturation, intraoperative adverse reactions and postoperative related complications, were recorded at each period, and to evaluate the anesthesia effect.Results:All patients were successfully completed surgery, anaesthesia effect was satisfied. The HR and MAP were lowered with different degrees at the point of T1 (there was no statistically significant difference between the groups, P> 0.05), The GLU, CORT and CA were significantly elevated (there were statistically significant differences between the groups, and the respective P values were listed below, P< 0.001; P= 0.005; P< 0.05); The incidence rates of intraoperative hypotension and bradycardia of group I were significantly lowered than group II (P< 0.05); The SpO2, GLU and CORT of groups had interaction effects at each time (the respective F and P values were listed below, F= 6.851, P= 0.002; F= 12.869, P< 0.001; F= 4.884, P= 0.025), the SpO2 of group II was obviously lowered than group I (P= 0.001) and the CORT of group II was obviously rose than group I (P= 0.006) at the point of T2; There were more postoperative nausea, vomiting, sore throat, dizziness, drowsiness and pulmonary infection in group II at the point of T2 (P< 0.05).Conclusion:The anaesthesia effects of the unilateral lumbar hemp using hypobaric bupivacaine and general anesthesia were all satisfied, but the unilateral lumbar hemp using hypobaric bupivacaine could more effectively regulate the stress response of elderly hip replacement.
