Objective: To analyze the relationship between the abnormal discharge site of video EEG and imaging in patients with brain tumor and epilepsy.Methods:Review the second hospital,hebei medical university epilepsy surgery in April 2018-February 2019 brain tumors with epilepsy 27 cases,all cases met in 2017,the international union of antiepileptic about epilepsy and epilepsy syndrome classification definition of comprehensive focal epilepsy,epilepsy,a full merger focal epilepsy,comprehensiveness,or unknown origin of focal epilepsy that refer to can't clearly determine the origin of attack is focal or comprehensive epilepsy,reviewing patients were performed surgical treatment(biopsy,and surgery).Pathology confirmed brain tumor.Before the operation,EEG machine was used for 2-hour video EEG,international 10-20 international lead system was used,and sphenoid electrode was used.On imaging examination,the location of 10-20 points in the EEG system was roughly marked at the occupied boundary determined by nuclear magnetic T2 FLAIR.The relationship between abnormal discharge and lesion at T2 FLAIR display was analyzed in each patient.Results:1.Imaging results: 27 patients underwent nuclear magnetic examination to determine the lesion boundary by T2 FLAI,16 cases of single brain lobe,including 7 frontal lobe,7 temporal lobe,2 parietal lobe;cumulative two lobe 7 cases,including occipital left buckle belt-corpus callosum in 1 case,double in 2 cases,dome in 1 case,right iliac island in 2 cases,left frontal iliac in 1 case,cumulative three cerebral lobe right forehead,left forehead Top 1 case..2.EEG results: EEG amplitude: Type I: no discharge.Type II: slow wave.Type III: ratchet and ratchet slow wave.Type IV: sharp and slow sharp waves.Two hours of video eeg was recorded in 27 patients,and the eeg was completely normal(type I)in 4 patients accounting for 14.8% of the total.The 23 cases with abnormal eeg accounted for 85.2% of the total number of cases,among which 10 cases showed slow wave(type II),8 cases showed slow wave(type III),and 5 cases showed slow wave(type IV).There were 17 cases of unilateral eeg abnormality and 6 cases of bilateral abnormality.3.Relationship between nuclear magnetic and electroencephalogram(EEG): two relations are divided into,compatibility: image boundary is greater than the eeg abnormalities boundary,intersect: less than eeg abnormal image boundary and have overlapping part,adjacent: image boundary with abnormal electroencephalogram boundary is clingy,apart: abnormal electroencephalogram boundary and abnormal image boundary between interval and less than the distance between two electrodes,far: abnormal electroencephalogram boundary and abnormal image boundary between interval and greater than the distance between two electrodes.The range of nuclear magnetic field is represented by the number of electrodes,and eeg by the number of electrodes.The relationship between the two was not evaluated in 4 cases with completely normal eeg.Among the remaining 23 cases,imaging suggested that the lesions were located on the left side in 11 cases,on the right side in 13 cases,and on both sides in 3 cases.There were 17 cases of abnormal eeg and imaging lesions located on the same side,accounting for 73.9% of the cases of abnormal eeg,among which 9 cases were abnormal on the right side and 8 cases were abnormal on the left side.Eeg showed bilateral abnormality in 6 cases and could not provide lateral information.In the number of abnormal eeg cases,the proportion is 8.7%,10 cases are intersects,43.5%,8 cases are adjacent,34.7%,2 cases are separated,8.7%,and 1 case is distant,4.3%.Conclusion:1.Two hours before operation,video EEG combined with imaging examination can improve the accuracy of locating epileptic foci in patients with brain tumor complicated with epilepsy.2.Video EEG indicated that most of the abnormal areas were around T2 Flair.3.Brain tumor with epilepsy,2 hours video EEG most of the manifestations of abnormal.
