Objective: To investigate the applied value of VEEG and MRI in thesecondary epilepsy, and evaluate the relationship between them.Methods: Routine EEG,24 hour dynamic EEG,VEEG and MRI orCT examinations were performed in 142 patients with secondary epilepsywas, and the positive rate of detecting eleptiform wave were comparedamong routine EEG, dynamic EEG and VEEG. The findings of initialperiod of seizure, intercritical VEEG and clinical manifestations wereanalyzed in patients with secondary epilepsy, and the imagingabnormality were then compared among them so as to find out therelationship between the epileptogenic focus and the pathological focus.Results:1,The positive rate of detecting eleptiform wave by VEEG wasobviously higher than that by routine EEG in 142 patients with secondaryepilepsy, which had significant difference. The positive rate of detectingeleptiform wave by VEEG obviously outweighed that by 24 hoursdynamic EEG.2,Spike (sharp) wave were caught in 99 patients with secondary epilepsy by intercritical VEEG, thus we often take it for granted that thebeginning of spike (sharp) wave was the origin of epileptogenic focus andthen we can find the epileptogenic focus.3,Comparing with the findings of initial period of seizure andintercritical VEEG, VEEG can get information about the epileptogenicfocus for 80.28%patients (114/142), the initial period of seizure wasregarded as the epileptogenic focus when it was not in accordance withintercritical VEEG。4,Among the 142 patients with secondary epilepsy, the imaging ofsecondary epilepsy differs from each other, which thus caused differenttypes of seizure.5,Aggregate analysis of VEEG and imaging findings, in 106patients, epileptogenic focus were coincident with pathological focus, andthe concordance rate was 74.64%. In 3 patients, epileptogenic focus wereon the contrary with pathological focus while 8 cases suspicious ofbilateral loci yet showed unilateral focus by imaging.Conclusion:1. During the diagnostic period of pathogenic location ofepileptogenic focus, most secondary epileptic patients can get lesion-sidelocation information about epileptogenic focus by analyzing the VEEG inthe interval and initial period of seizure and seizure symptoms,2. VEEG can obviously increase the positive rate of detecting epileptic discharge.3. Imagines play an important role in finding the pathologic locus inmost patients with secondary epilepsy.4. The location information of epileptogenic focus can be moreaccurate by analyzing both of the VEEG and imaging.
